Svrha: procjena nosača i status fiksnih protetskih nadomjestaka kod pacijenata koji su zatražili njihovu zamjenu. Materijali i metode: U Zavod za protetiku Stomatološkog fakulteta Sveučilišta Ege poslano je 115 ljudi kako bi im se s razlogom skinuo FPN. Raspon godina bio je između 35 i 76 (53,98±8,55). Kategorije pregledanih fiksnih nadomjestaka bile su sljedeće: 15 (13%) potpunih metalnih krunica, 63 (54,8%) metalno-akrilatnih mostova i 37 (32,2%) metalno-keramičkih. Bio je zabilježen i broj članova, položaj i vrsta restoracija u zubnom luku te starost. U postupku procjene okluzije koristili smo se kriterijima USPDH-a, a za procjenu kliničke kakvoće restoracija uprabljene su ocjene iz sustava CDA. Rezultati: Pregledano je ukupno 100 mostova sa 727 članova, 464 nosača i 263 međučlana. Prosječna im je iznosila je 6,53 (±7,04) godina za krunice, 8,16 (±9,28) za metalno-keramičke mostove i 10,00 (±6,21) godina za metalno-akrilatne mostove. Prosječna starost djelomičnih fiksnih proteza bila je znatno veća u metalno-akrilatnoj skupini negoli u metalno-keramičkoj (p=0,017) i kod potpuno metalnih krunica (p=0,015). Kliničkom procjenom otkriveno je da su parodontno zdravlje (p=0,011), estetika (p=0,000) i higijena (p=0,002) bili na nižoj razini kod metalno-akrilatnih DFP-a negoli kod drugih vrsta djelomičnih fiksnih proteza. Najveći gubitak proksimalnih kontakata dogodio se u skupini metalno-akrilatnih FPN-a (p=0,003). Zaključak: Prije skidanja djelomično fiksnih proteza ustanovljeno je pregledom i procjenom na temelju kriterija CDA-a da je ocjenu „neprihvatljivo“ za parodontno zdravlje, estetiku i higijenu dobilo više radova DFP-a iz skupine metalno-akrilatnih mostova negoli iz onih skupinemetalno-keramičkih. |

| Abstract: |
The question of whether to cement or fix a bridge on implants by means of screws is still one of the frequent dilemmas in implantology. In the early days of implantology fixation with screws was considered the only adequate solution. With the development of the profession, primarily biomechanics and componentistics, cementing has increasingly become the method of choice. Both possibilities are described in the presentation, with emphasis on the features in clinical application. Fixation with screws represents a simple, fast and inexpensive method, while cementing is more suitable in aesthetically demanding cases and in the case of extreme vestibular inclination or disparallelism of the implant. Two typical cases are shown of distal bridges in the upper jaw, of which one is cemented and the other fixed with screws. |

| Abstract: |
One of the methods of implantoprosthetic therapy of complete edentia/edentulousness of the lower jaw is the fabrication of a fixed bridge on implants. Such therapy assumes the placement of 4 to 6 implants in the interforamen area and construction of bridges on the implants. Such bridges are usually fixed with screws, rarely cemented, and distal cantilever are the rule. The basic problem in screwed fixed superstructures is passivity. Because of the technological process of constructing metal superstructures, thermal changes are not unusual, which results in the occurrence of tension when fixing on the implant. Any tension is harmful and finally leads to mechanical complications of the superstructure. Another problem is distal cantilever. As a general rule the length of the cantilever amounts to 2X, in which X represents the vertical length (space) between the last and the penultimate implant. This means that cantilever can be longer, which is better spatial arrangement of the implants. Other less important problems with cantilevers are of a technical nature and can be avoided by correct shaping of the superstructure. The presentation gives an evaluation of the success of therapy with screwed fixed bridges in the lower edentulous jaws. The period of monitoring ranges from 3 to 5 years, with analysis of complications in the same time period. Mechanical and biological complications are separately analysed. The frequency and character of mechanical complications are analysed. The results of the research provide guidelines for safer and more successful clinical work with such types of prosthetic superstructures on implants. |

| Abstract: |
Epidemiological studies on the condition of existing or the need for new fixed prosthodontic appliances, with the purpose of planning required prosthetic teams, improved materials and methods, are essential in modern prosthodontics.The aim of this study was to evaluate the aesthetic material used in patients with fixed prosthodontic appliances in the Metkovi} region and determine the frequency of crowns and bridges in relation to age, gender, level of education, employment, socio-economic status and frequency of regular visits to the dentist. examination The study included 212 patients, wearing fixed prosthodontic appliances, who were examined in the dental office (clinic) at the Metkovi} Medical Centre during 2001.Descriptive statistics and (2 tests were used during data analysis. The following conclusions were made: 1. The older group of patients had significantly more bridges than crowns (p<0.01). There was no significant difference between gender with regard to the distribution of crowns and bridges (p>0.05), or between patients of different levels of education (p>0.05). Retired patients had significantly more bridges than crowns compared to employed or unemployed patients (p<0.,05). Patients who visit their dentist regularly (once a year or more often) had significantly more crowns than bridges compared to the patients who visit their dentist irregularly or when in pain (p<0.01). This group (irregular visits to their dentist) had more teeth extracted and consequently bridges than the patients who regularly visit their dentist . There was no significant difference between patients with regard to crown and bridge prevalence, depending on the age when they first visited the dentist (p>0.,05). Less educated patients had more fixed prosthodontic appliances older than 10 years than better educated patients (p<0.01). 2. Relatively high frequency of porcelain (64%) was recorded in comparison to acrylic material (36%). There was no difference in gender depending on the material used (p>0.05). Patients older than 60 and less educated patients had more acrylic appliances than younger patients (<39 years) who had mainly only ceramic appliances (p<0.01). |

The aim of this study was to evaluate the type and the aesthetic material in relation to age, gender, level of education, employment, socio-economic status and frequency of visits to the dentist. The examination was performed on 212 patients who had a fixed prosthodontic appliance for more than a year (55% males and 45% females, age 18-80 yrs.). The following conclusions were made: 1. The older patients and retired patients had significantly more bridges than crowns (p<0.05). There was no significant difference between gender and patients with a different level of education in the distribution of crowns and bridges (p>0.05). Patients who visit their dentist regularly have significantly more crowns than bridges thanpatients who visit their dentist irregularly or when in pain (p<0.01). Age or the first visit to the dentist made no significant influence on crown and bridge prevalence (p>0.05). 2. Almost all fixed prosthodontic appliances older than 10 years were made of porcelain (98%), while acrylic veneer crowns were more frequent in appliancesolder than 10 or 15 years (p<0.01). Relatively high frequency of porcelain (64%) was recorded in comparison to acrylic material(32%) or chromasite (4%). There was no gender difference with regard to the material used (p>0.05). Patients older than 60 years had more acrylic material compared to younger patients. While patients younger than 39 years had almost exclusively ceramicappliances (p<0.01). Less educated patients had more acrylic veneer appliances. Employed patients had significantly more ceramic appliances than retired patients. |

INTRODUCTION: The demand for all-ceramic restorations has increased substantially because of their esthetics and biocompatibility. However, the indication for a fixed partial denture has been limited by numerous problems such as low breaking strength of conventional dental ceramic and complex manufacturing techniques. Procera AllCeram is a CAD/CAM system for completeceramic restorations with a dry-sintered high-purity aluminium oxide core.Purpose: The aim of this study was to determine whether the Procera ceramic fixed partial denture is an acceptable treatment modality.Material and methods: Nine patients were treated with a total of ten fixed partial dentures of three units. The restorations were constructed with bilateral support and one pontic, accordingto the Procera AllCeram technique. The California Dental Association quality evaluation system was used for assessment of marginal integrity and esthetics after two years. RESULTS: Nine of ten fixed partial dentures (90%) showed no defects and were functioning well after two years. No caries or signs of gingivitis or periodontitis exceeding those found.CONCLUSION: The study confirms that for the observation period of two years, fixed partial dentures made by the Procera AllCeram method seem to be an acceptable treatment alternative. |

| Abstract: |
Two concepts of prosthetic restorations exist for the edentulous jaw: the fixed prosthesis (mostly screw retained) and the overdenture. It appears that overdentures are preferably placed in old patients and in compromised situations. Since the eighties, a series of studies - including longterm results - have demonstrated the reliability of treatment with fixed prostheses for the upper and lower jaw. An increasing number of studies on mandibular overdentures supported by only two implants give evidence of the effectiveness of this treatment modality. Comparable data for maxillary overdentures are still missing. While dentists tend to base the selection of the prostheticdesign on the number of implants that can be placed, other criteria have to be considered: esthetic appearance, facial morphology and restitution of lost hard and soft tissues, costs of implant-prosthodontic treatment, stability of the prostheses, complications and adjustmentsrequired, assessment of individual needs. From an economic point of view overdentures supported by two to four implants might be preferred. Prosthetic methods in general and related to implants are not evidence based. They relay on clinical experiences, patients’ demands technical considerations and reports of success and failure. However, from clinical experience, well-designed clinical concepts have evolved and the benefit of the patients concerned appears to behigh and obvious. The lecture will discuss the use of implants forprosthodontic rehabilitation in the completely edentulous jaw. Indications and various types of removable prostheses are presented and variations of design discussed. Biomechanical aspects of fixation and stabilization of prosthesis complete the overview. |

| Abstract: |
Development of oral biomaterials has been focused on composites of various kinds. From the prosthodotic point of view, the most promising composites are fibre-reinforced composites (FRC). The use of FRCs in prosthodontics is rapidly growing at the moment and their applications are in FRC full coverage crown bridges, surfaceretained bridges, inlay retained bridges and in root canal posts. Interestingly, the currently available data of biomechanics and preliminary clinical findings suggest that combination of retentive / adhesive elements of different kind of bridges can now be combined to a single FRC bridge/restoration. By this, odontological and subjective needs of the patient toward fixed prosthetic therapy can better be taken into consideration. This treatment philosophy is called “the dynamic treatment approach”. Successful use of FRC in the dynamic treatment approach put some demands on FRC material. The use of FRC should be easy to use for dental technician and for dentist, the adhesional behaviour of FRC to composite resin luting cement should be good enough, and finally, the biomechanicalproperties of the FRC should correspond the needs of the masticatory system. Recent development in the field of dental FRC materials have been able to resolve many of the aforementioned questions. These aspects with clinical examples will be demonstrated in the lecture. |

| Abstract: |
The aim of this study was to screen patients with fixed prosthodontic appliances, in the mouth for a period of 5 years or more, for root caries and pocket formation, deeper than 3 mm. The aim was also to determine the difference between frequency of pathologic findings for different materials of fixed prosthodontic appliances (FPA), or between abutments and natural teeth. A total of 260 patients (55% women and 45% men) aged from 25 to 70 years, who had a fixed prosthodontic appliance for a period of 5 years or more in their mouth and who were fully satisfied, participated in the study. A total of 260 orthopantomographs were analysed with 2265 teeth, 864 were abutments of the bridges, 407 crowns and the rest natural teeth. Radiographs were analysed for root caries and pocket formation. The results reveal relatively high incidence of pathologic findings (caries, periodontal pockets > 3 mm) in patients with FPDs older than 5 years. Periodontal pockets were more frequent than cervical caries. The highest frequency of pathologic findings was registered for abutments with metal acrylic crowns, and the lowest frequency of pathologic findings was registered for abutments with metal-ceramic crowns (p<0.05). There were no significant differences between abutments with metal-ceramic crowns and non-abutment teeth (p>0.05). There were no significant differences in caries incidence between the examined teeth (p>0.05). Non-abutment teeth and abutments of metal-ceramic crowns had significantly lower frequency of pockets deeper than 3 mm than abutments of metal acrylic and fullmetallic crowns (p<0.05). |

| Abstract: |
A technique of etching was employed in 50 patients for bridge retention. The Rexillium III (Jenevie Industries, Wallingford Co.) alloy and Comspan bond (L. D. Caulk Co., Milford Del.) were used. Therapeutic results obtained with adhesive (etched) bridges depend on a variety of factor: careful and accurate indication, precise work plan, minimal preparation of the supporting tooth, appropriate method of retention of the attachment metal portion, and the type of bond and of dental alloy. Clinical application of bond cast bridges has a quite good prognosis. A prolonged period of observation should provide more data on the behavior of such appliances in the oral cavity and allow us evaluations. |

| Abstract: |
In fixed prosthodontics, analysis of the distribution of basic prosthetic appliances according to sex, dental arch and particular types of teeth, is quite understandably needed. Therefore, these relations were analyzed in a sample of young adults. The aim was to find out the trends prevailing in our population, that may have proved useful in choosing appropriate measures in prosthetic therapy. Aimed clinical studies were conducted in 5665 students, 3078 females and 2587 males aged 18-20 years. The results obtained pointed to a significant difference in the prosthetic appliances between the upper and lower jaws (89.36% and 10.64%, respectively). Of the total of 1306 appliances in both jaws, 994 (76%) referred to anterior teeth, including first premolars, indicating the prevalence of esthetic requests and indications as compared to the functional ones. According to age, 57% of the total number of prosthetic appliances were made for females and 43% males. |
