The object of this investigation was to estimate the possible premalignant potential of lesions of oral lichen ruber (OLR) by determining the intensity of expression of tissue antigen c-erbB-2 in relation to the clinical form and clinical status of the lesion. Comparison of the tissue expression of antigen cerbB- 2 was carried out between erosive (LRE) and planus form (LRP) oral lichen, and comparison according to the intensity of inflammation and hyperceratous lesions.The investigation included 30 patients with clinical and histopathological diagnosis of OLR. The results obtained were compared with a control group comprising 15 patients with a diagnosis of oral leukoplakia (OL), verified as leukoplakia simplex. Tissue antigen c-erbB-2 was demonstrated by APAAP and LSAB immunohistochemical methods, following prior analysis of biopsy samples in a microwave oven. The reaction showed specific mosaicism, in both intensity and topography occurrence. Positive reaction to this tissue antigen was observed, primarily in the spinous layer of cells of the oral epithelia, while negative reaction occurred in the basal layer of cells. The reaction was intensively positive in the tonofibrile cells of the spinous layer. No difference was observed in the intensity of the reaction to this tissue antigen between erosive and planus form of oral lichen. The intensity of the reaction to tissue antigen c-erbB-2 did not correlate with the intensity of the inflammation, although it positively correlated with the intensity of the hyperkeratose lesion.It can be said that expression of tissue antigen c-erbB-2 in ORL lesions was changed, which indicates their changed nature and specific premalignant potential. |

Oral lichen ruber (OLR) is a chronic, mucocutaneous autoimmune disease. The etiology of this oral disease is still not clear. It is assumed that some uncharacteristic antigens adher to keratinocyte surfaces and in that way change antigenic structure of keratinocytes, which can influence autoimmunity reaction. The aim of this investigation was to detect how much that hypothesis can be included in classical pathogenesis of OLR. We took cytological smears (N = 30) of OLR lesions (clinical and pathohistologically confirmed) before and after antibiotic therapy and comparedthem with the control group (N =30) of healthy subjects. Detection of that observation was done by Gramm and light microscopy method. Results showed a significant lower number of streptococcus and other bacterial microorganisms, which adhered to keratinocytes surfaces, after antibiotic therapy and compared with the control group. The clinical status was also better. These results can be a key step in the pathogenesis of OLR, and the mechanism and role of bacterial adherence to oral keratinocytes can be considered. |

| Abstract: |
Oral lichen ruber (OLR) is a chronic, mucocutaneous, autoimmune disease with unclear etiology. There are many etiology factors which can be included in the pathogenesis of this oral disease. results of new investigations describe liver diseases, especially HCV-infection in the etiology and pathogenesis of OLR. The aim of this investigation was to detect the relationship between OLR and HCV-infection. We performed a serological investigation of anti-HAV, anti-HBV and anti-HCV markers. Results showed a connection between OLR and viral diseases, especially HCV-infection. That results need complete laboratory tests for patients with OLR, which include to detect specific antibody to viruses antigenes especially anti HCV, which can be in the etiology and pathogenesis of this disease. |

Oral Lichen Ruber (OLR) is a precancerous lesion of mucous membrane, according to WHO criteria.Increasing frequency of malignant alteration was observed among atrophic, erosive and ulcerous lesions. The aim of this investigation was to define possible malignant potential of OLR lesions by determining the expression intensity of PCNA and Ki-67 antigens, as indicators of activity and proliferation of these lesions. The aim was to determine expression of these antigens in lesions of OLR and compare with some clinical forms of these lesions. The investigation included 30 patients with OLR (N = 30) with clinical and pathologically confirmed diagnosis and a control group of patients with lesion of oral leukoplakia (OL) (N = 15). Clinical tests based on standard clinical tests and methods and biopsy samples were observed by immunohistochemical methods by specific monoclonal antibody. Results showed more statistically significant expression of PCNA and Ki-67 in OLR lesions than in OL lesions and expression was stronger in erosive than in planes forms of OLR.It can be concluded that such altered expression of PCNA and Ki-67 antigens in OLR lesions points to the altered nature of those lesions with the potential to undergo malignant transformation. |

Immunological disorder in oral lichen ruber (OLR) is related to cell mediated immunological reaction which can imply various immunological disorders in the oral mucous membrane. Changed immunoreaction characterized by suppressed function of phagocyte and NK-cells reduce the possibility of elimination of the bacterial antigens with possible resultant onset of autoimmune disorders, stressing the importance of nonspecific immunity. Evaluation of cell mediated immunity is based on phenotypic and functional analysis of peripheral blood cells by monoclonal antibodies. T-cell mediated suppression has an important role in maintaining its own tolerance, so the disorder of T-cells is included in the pathogenesis of autoimmune diseases. A consequence of this could be activation of immature T-lymphocytes, causing cellular autoreactivity in OLR. Disorder of cell mediated immunity could reflect on the humoral immunity condition. Humoral immunity in patients with OLR has been reported, but with inconsistent results. The result of humoral immunity is not specific, but is a consequence of the changed immunoreaction of patients with OLR caused by other immunological disorders and diseases where oral lichen could manifest |

| Abstract: |
Oral lichen rubber (OLR) is a frequentchronic mucocutane autoimmune disease in the population. OLR mostly affects middleaged and older individuals. Women make 2/3 of patients. Etiology of the disease is still unexplained, with numerous etiologic factors being the possible cause of the onset of this disease. Genetic predispositions are the most frequently quoted cause together with the local use of specific dental materials, systematic use of specific drugs in basic disease therapy, infection, results of autoimmune disease, and disorders such as diabetes mellitus, chronic liver disease, hypertension, and emotional stress. All the above mentioned factors can induce a sequence of immunologic events and reflect on the immunological state of the patient.Thus, induced sequence of immunological events is based on cell mediated immunity, including interactivity of oral epithelium cells and the immunological cell mediated by activity of adhesion molecules, and is behind the pathogenetic mechanism of OLR. |

| Abstract: |
A survey, which consisted of 21 questions, was conducted during 1999/2000 on 135 dentists in Croatia. Our aim was to objectify dentists’ knowledge of HIV infection. A questionnaire was divided into four groups, as following: general knowledge of HIV, protection and infection control, risks while treating HIV infected patients, and willingness to treat HIV infected patients. Only 40% of surveyed dentists know basic facts of HIV, oral manifestations and modes of transmission, and 50% of all surveyed dentists use protective masks, gloves and eyewear. More than 49% overestimate risks while treating HIV infected patients. More than 55% of all surveyed dentists are willing to treat HIV infected patients. Results of this survey show informational gaps in dentists’ knowledge of HIV infection and highlights the need for additional lectures on HIV infection. |
