Abstract: |
The aim of this study was to evaluate the effect of miconazole gel delivered on a custom thermoformed tray as an overnight drug holder appliance in denture stomatitis treatment, in comparison with classic application as recommended by manufacturer. The investigation included 44 upper denture wearers, diagnosed with denture stomatitis. The experimental group (n = 22) applied 2% miconazole gel in custom tray for overnight wear. The control group (n = 22) applied 2,5 ml of the same drug 4 times a day in their mouth. Intensity of infl ammation and infection was evaluated. The clinical tests that were used were: infl ammation intensity according to Newton, extent of palatal mucosal erythematous surface below denture, digital thermometry, and swabs for Candida albicans cultures. Oldness and hygiene status of dentures were recorded, as well as habit of overnight denture wearing. The investigation revealed that denture stomatitis healed faster and better in the experimental group compared to control group. Greater Candida albicans culture colony count reduction was observed in experimental group, which suggests more effi cient elimination of Candida albicans from the mouth with novel method. This method motivates patients to adhere to treatment and appliance hygiene instructions. Custom splint as occlusive drug delivery showed to be more effective and more convenient for the patients than the same antimicotic drug, classically delivered, which might be clinically relevant in treatment of denture stomatitis. |

Abstract: |
The aim of this study was to examine the effects of a 10% carbamide peroxide bleaching gel (VivaStyle, Ivoclar Vivadent, Schaan, Liechtenstein) on periodontal health. Ten participants, 22 to 29 years of age, with clinically healthy periodontal tissues and a wish to change the shade of their teeth, participated in this study. Subjects were treated with 10% carbamide peroxide for one hour daily during 12 days. The volunteers were instructed to keep a diary of possible disturbances during the teeth whitening procedure. At baseline, on 6th and on 12th day API score and PBI score were measured and clinical photograph of teeth in intercuspidation relation was taken. All teeth demonstrated change of at least one shade. There was no change in either plaque accumulation, or gingival inflammation during the teeth whitening procedure. This teeth whitening procedure seems not to have any influence on the periodontal health. |

Abstract: |
Nonaromatic naphthalane (NAVS) is a specific fraction of Croatian oil, extremely rich in steranes from which the aromatic system is removed. Steranes are attributed with bioactivity similar to steroid hormones, modulators of tumour growth. Encouraged by the results of earlier in vitro and in vivo studies on the inhibitory effect of NAVS on the growth of planocellular carcinoma, we carried out a study on its effect on oral planocellular carcinoma (OPCC) in mice.Aims: 1) To test the validity of the simple experimental model OPCC, 2) to test possible antiproliferative effect of NAVS on the above model by monitoring tumour growth, 3) to test the antineoangiogenic effect of NAVS to explain the possible antiproliferative effect, and to estimate the possibility of crisis reactivity of anti-human immunohistochemical markers with mice tissue. A suspension of 100 μl s 105 SCC VII cell was inoculated intraorally under the buccal mucous membrane in 48 syngeneic C3H mice. Seven days after inoculation the animals were divided in six equal groups and the mice, depending on the group, were intratumorously injected with 100 μl of the following substances: paraffin oil (PO) as a negative control, NAVS (in one group 7 days, and in the second group 14 days, after inoculation of the tumour), 1.25 dihydroxyergotamine (1.25-D3) as a positive control, and a combination of NAVS with 1.25 D3 and PO with 1.25- D3. Tumour growth was monitored weekly by measuring with callipers. The animals were sacrificed 1, 2, 3 and 4 week after application of the tested substance. Histological specimens of explanted tumours were stained with hematoxylin-eozine, and immunohistochemically with anti-CD34 antibodies for estimation of tumour neoangiogenesis. Compared with the PO group, tumour growth and angiogenesis were decreased in the 1.25-D3 and NAVS groups. NAVS probably reduced growth by OPCC inhibition of vascular proliferation, needed for tumour growth. |

Abstract: |
Data can be found in the literature on the roles of interleukin 6 (IL-6) and basic growth factor of fibroblasts (bFGF) in patients with oral carcinomas, although there are no data on salivary IL-6 and bFGF in such patients. The aim of this study was to determine the values of salivary and serum IL-6 and bFGF in 35 patients with planocellular carcinoma of the oral cavity, aged 40 - 73 years (mean age 54 years). A control group consisted of 23 healthy participants, aged 25 years. The data obtained were analysed by Mann-Whitney U test and values of less than 0.001 were considered statistically significant. Serum values IL-6 and bFGF did not significantly differ between the examined and the control group. Increased values of salivary IL-6 and bFGF were determined in the patients with planocellular carcinoma compared with the control group (p<0.001). We can conclude that the increased values of salivary IL-6 and bFGF in patients with planocellular carcinoma originated from local production. |

Abstract: |
The object of this investigation was to estimate the possible premalignant potential of lesions of oral lichen ruber (OLR) by determining the intensity of expression of tissue antigen c-erbB-2 in relation to the clinical form and clinical status of the lesion. Comparison of the tissue expression of antigen cerbB- 2 was carried out between erosive (LRE) and planus form (LRP) oral lichen, and comparison according to the intensity of inflammation and hyperceratous lesions. The investigation included 30 patients with clinical and histopathological diagnosis of OLR. The results obtained were compared with a control group comprising 15 patients with a diagnosis of oral leukoplakia (OL), verified as leukoplakia simplex. Tissue antigen c-erbB-2 was demonstrated by APAAP and LSAB immunohistochemical methods, following prior analysis of biopsy samples in a microwave oven. The reaction showed specific mosaicism, in both intensity and topography occurrence. Positive reaction to this tissue antigen was observed, primarily in the spinous layer of cells of the oral epithelia, while negative reaction occurred in the basal layer of cells. The reaction was intensively positive in the tonofibrile cells of the spinous layer. No difference was observed in the intensity of the reaction to this tissue antigen between erosive and planus form of oral lichen. The intensity of the reaction to tissue antigen c-erbB-2 did not correlate with the intensity of the inflammation, although it positively correlated with the intensity of the hyperkeratose lesion. It can be said that expression of tissue antigen c-erbB-2 in ORL lesions was changed, which indicates their changed nature and specific premalignant potential. |

Abstract: |
Oral lichen planus (OLP) is a chronic inflammatory disease whose etiology has not yet been identified. It is characterizied by immunoreactivity directed against basal keratinocytes and mediated by T-lymphocytes. In health, salivary peroxidase together with other salivary antimicrobial enzymes maintains the ecological balance in theoral cavity. The aim of this study was to evaluate salivary peroxidase levels in patients with OLP. In 30 patients with OLP, salivary peroxidase levels in whole unstimulated saliva were determined and compared to 28 healthy controls. Each participant collected saliva using the simple method of spitting while sitting unstimulated whole saliva intocalibrated tubes during five minutes between 8-11 A.M. Salivary samples were then centrifuged on 800 turns during ten minutes and supernatans were frozen at -70°C until analysis. Salivary peroxidase levels were determined according to Putter and Becker. Statistical analysis was performed using Student t-tes and p-values below 0.05were considered statistically significant. No significant differnces between patients with OLP and controls in salivary flow rate, as well as in salivary peroxidase levels were found. We can conclude that salivary peroxidase levels in patients with OLP did not have an impact on changes seen in the oral cavity of these patients. |

Abstract: |
Pemphigus vulgaris is a rare, potentially lethal chronic autoimmune vesiculobullous disease of the skin and the mucous membrane. Most commonly it involves patients in their fifties and sixties and extremly rare in adolescence. Eighty to ninety percent of patient with pemphigus vulgaris develop oral lesions and 60% of cases oral lesions are the first sign. It is common for the oral lesions to be present up to 4 months before the skin lesions apper. The autors present a case of pemphigus vulgaris, which developed initially in an 18-year old girl and was confined to her mouth. Clinical and histopathological findings, also direct and indirect immunofluorescence studies of the oral mucosa confirmed the diagnosis of pemphigus vulgaris, with positive antibodies to desmoglein 3 and 1. The patient was successfully treated with systemic and topical corticosteroids in association with miconazole. The patient is treated with low-dose systemic corticosteroids, and during 11 years skin involvement has not been found. Recognition of oral lesions of adolescent pemphigus vulgaris by the clinician, its early diagnosis, and prompt therapy are important in order to eliminate or reduce the severity of further intraoral or extraoral involvement. |

Abstract: |
Xerostomia, i.e. oral dryness, is mainly caused by side effects of medications, immunologic diseases, e.g. Sy. Sjoegren, and radiation treatment for oral and neck cancer. There are variable, although far from satisfactory, treatment options, including that of muscarinic receptor agonist pilocarpine-hydrochloride. Pilocarpine treatment of salivary hypofunction is not yet an established method in croatia. The aim of this preliminary study was to assess the autcome of oral administration of pilocarpin on minor salivary glands and whole saliva production, as well as to monitor possible side effects and patients subjective responses to treatment. Is was a single blind-placebo controlled trial. Ten patients suffering from xerostomia underwent this trial. Patients with cardiac or pulmonary diseases were excludet, as well as those that did not show any increase in salivary flow rate after administration of ascorbic acid solution (chemical stimulant) at inclusion testing one week prior to the experiment. The experiment comprised 7 repeated measurements in time intervals of 15-30 min., from the baseline up to 2 h after single oral administration of 5 mg of pilocarpine hydrochloride ( 5 drops of slution, Pilokarpin 2%, pliva, Croatia). The flow rate of minor salivary glands was measured on the lower lip and palate by means of Periotron (Oraflow Inc., Plainview, USA) and whole saliva was collected in calibrated test tubes . A simple questionnaire was filled in to record patients subjective perception of treatment. The same procedure was repeated one week later, when dexpanthenol drops (DPanthenol, Ljekarne Zagreb, Croatia) were used as placebo instead of pilocarpine.Pilocarpine increased salivary output and gave symptomatic relief in every patient. The differences in salivary output were significant both compared to baseline and to placebo controls (p = 0,01). There were no side effects during the trial, either after administration of pilocarpine; or placebo. After increasing salivary flow rate the action of pilocarpine decrease in time, which was recordable during the observation session.Respecting possible contraindications and side effects, according to our finding pilocarpine should be regulary used in treatment of xerostomia. |

Abstract: |
Oral lichen ruber (OLR) is a chronic, mucocutaneous autoimmune disease. The etiology of this oral disease is still not clear. It is assumed that some uncharacteristic antigens adher to keratinocyte surfaces and in that way change antigenic structure of keratinocytes, which can influence autoimmunity reaction. The aim of this investigation was to detect how much that hypothesis can be included in classical pathogenesis of OLR. We took cytological smears (N = 30) of OLR lesions (clinical and pathohistologically confirmed) before and after antibiotic therapy and comparedthem with the control group (N =30) of healthy subjects. Detection of that observation was done by Gramm and light microscopy method. Results showed a significant lower number of streptococcus and other bacterial microorganisms, which adhered to keratinocytes surfaces, after antibiotic therapy and compared with the control group. The clinical status was also better. These results can be a key step in the pathogenesis of OLR, and the mechanism and role of bacterial adherence to oral keratinocytes can be considered. |

Oral lichen ruber (OLR) is a chronic, mucocutaneous, autoimmune disease with unclear etiology. There are many etiology factors which can be included in the pathogenesis of this oral disease. results of new investigations describe liver diseases, especially HCV-infection in the etiology and pathogenesis of OLR. The aim of this investigation was to detect the relationship between OLR and HCV-infection. We performed a serological investigation of anti-HAV, anti-HBV and anti-HCV markers. Results showed a connection between OLR and viral diseases, especially HCV-infection. That results need complete laboratory tests for patients with OLR, which include to detect specific antibody to viruses antigenes especially anti HCV, which can be in the etiology and pathogenesis of this disease. |

Abstract: |
Oral Lichen Ruber (OLR) is a precancerous lesion of mucous membrane, according to WHO criteria.Increasing frequency of malignant alteration was observed among atrophic, erosive and ulcerous lesions. The aim of this investigation was to define possible malignant potential of OLR lesions by determining the expression intensity of PCNA and Ki-67 antigens, as indicators of activity and proliferation of these lesions. The aim was to determine expression of these antigens in lesions of OLR and compare with some clinical forms of these lesions. The investigation included 30 patients with OLR (N = 30) with clinical and pathologically confirmed diagnosis and a control group of patients with lesion of oral leukoplakia (OL) (N = 15). Clinical tests based on standard clinical tests and methods and biopsy samples were observed by immunohistochemical methods by specific monoclonal antibody. Results showed more statistically significant expression of PCNA and Ki-67 in OLR lesions than in OL lesions and expression was stronger in erosive than in planes forms of OLR.It can be concluded that such altered expression of PCNA and Ki-67 antigens in OLR lesions points to the altered nature of those lesions with the potential to undergo malignant transformation. |

Abstract: |
The Periotron micro-moisture meter (Oraflow, Plainview, USA) is an electronic instrument that has been designed for gingival vrevicular fluid (GCF), periodontal pocket fluid (PPF), salivary flow and saliva thickness measurments, by using a variety of paper collection strips. Collection strips (Periopaper strips, PerioCol strips, Sialopaper strips) are applied to the oral tissues of interest for a certain period of time, after which the are inserted between Periotron electrodes. The special insulation coating on the electrodes enables them to work as two plates of a capacitor, being able to measure submicroliter quantities of oral fluids. Periotron displays the numerical output on LCD. By inputting data obtained during prior calibration process into Periotron professional software for Windows, we design a standard curve, from which we interpolate actual fluid volume. We have designed a single blind placebo controlled study to evaluate possible presence of long-term effect of pilocarpine-hydrocloride (PHC) on salivary flow rate in patients with xerostomia. 12 patients suffering from xerostomia underwent this trial. We used strict inclusion criteria regarding the possible side effects. Six patients were instructed to self-administer 5 mh of PHC (5 drops of solution, Pilokarpin 2%, pliva Zagreb, Croatia), three times a day, for 7 days. Another 6 patients (placebo group) were instructed to take 5 dexapanthenol drops (D panthenol, Ljekarne Zagreb, Zagreb, croatia), 3 times a day for 7 weeks. The flow rate of minor salivary glands was measured on the lower lip and palate by means of Periotron and whole saliva was collected in calibrated test tubes. mesurements were repeated once a week. Each patient filled in an extensive Oral Health Impact Profile questionnaire (OHIP) at te baseline and after completion of PHC treatment to monitor oral health influence on patients lives. After 7 weeks of treatment the placebo group was swiched to PHC, but there no apparent need for switching pilocarpine group to placebo, because we found no improvement in salivary flow rate after 7 weeks of treatment. in the group previously taking placebo, there was also no improvement in salivary flow rate after taking PHC. OHIP findings were calculated and showed no statistically significnt improvement after 7 weeks of PHC administration. One patient from the pilocarpine group and one from palcebo group ( while taking placebo) reported side effects and discontinuited the trial. oral administration of PHC does not seem to produce log-term salivary flow rate increase, but rther immediate and shortr lasting improvement. Judging by Periotron and whole saliva scores, our results showed that PHC is not able to “cure” xerostomia after prolonged administration, i.e. there is no residual effect in salivary gland simulation once the drug is discontinued and eliminated. |

Abstract: |
The purpose of this lecture is to summarize data obtained from several experiments conducted by authors, regarding the composition of special Croatian petrol fraction reffered to as “Non Aromatic- Very rich in Steranes” (NAVS) naphthalane, as well as its potent-inflammatory and cellular growth controlling effect. NAVS is derived from ordinary brown naphthalane, particulaly rich in steranes, that has been famous for decades in the treatment of psoriasis vulgaris (Naftalan, Ivanić Grad, Croatia). NAVS was prepared by removing virtually all of its original polycyclic aromatic content (some of them are carcinogenic), which resulted in completely colourless transparent oil with a pleasant scent. Steranes were preserved and concentrated during the de-aromatisation process, as was proven by means of GC and GC-MS. These geogenic steranes have molecular skeletons analogous to bioactive compounds, such as corticosteroids and vitamin D. Due to these structure similarities, we can explain the beneficial affects in treating psoriatic patients with NAVS, which yielded promising results. NAVS treatment did not affect either haematological or biochemical findings. Further studies established In vitro dose dependant inhibition of planocellular carcinoma (SCC VII) cell proliferation, withhout interfering with nonmalignant fibroblast (L929) cells, thus showing selectivity in cell proliferation control. In vivo results showed marked delay in tumour growth. A rationale for that, besides cell proliferation control, authors see also in the antineoangiogenetic activity of NAVS (similar to vitamin D), wich is yet to bi proved through immunohistochemical studies. Future studies are planned and oral planocellular carsinoma animal models are being developed in order to introduce NAVS in the treatment of different conditions of oral mucosa, because the preparation is not only effective, but also highly applicable in oral tissues, due to its superior organoleptic properties. |

Abstract: |
The aim of this prospective study was to record the presence of Candida albicans yeast in a geriatric population, and to compare the results between denture wearers and non-wearers, in order to evaluate the role of dentures as a factor influencing Candida growth. We performed an oral examination on 217 institutionalized elderly people living in the hospice "Sv. Josip" in Zagreb, recorded dataregarding oral health and collected detailed information on denture wearing status. Furthermore, we performed oral smears for Candidacultures, cultivated on Sabouraud's dextrose agar (“Emmon's”) at 37°C for 48 hrs and counted the Candida colonies. Collected data were stored in the database created for this purpose in "Microsoft Access 2000". The number of Candida colonies from denture wearers andnon-wearers were compared (Man-Whitney U test, p<0.05), and the obtained results are discussed. |

Abstract: |
In 28 young, healthy individuals the quantity of salivary peroxidase was determined with the aim of evaluating normal ranges for salivary peroxidase, to determine whether any differences in SP values between unstimulated and stimulated whole saliva exist and to find any possible differences between sexes. We can conclude that no statistically significant differences between salivary peroxidase values in unstimulated and stimulated whole saliva were found. No statistically significant differences were found between female and male individuals in salivary peroxidase values, either in unstimulated or stimulated whole saliva. |

Abstract: |
A survey, which consisted of 21 questions, was conducted during 1999/2000 on 135 dentists in Croatia. Our aim was to objectify dentists’ knowledge of HIV infection. A questionnaire was divided into four groups, as following: general knowledge of HIV, protection and infection control, risks while treating HIV infected patients, and willingness to treat HIV infected patients. Only 40% of surveyed dentists know basic facts of HIV, oral manifestations and modes of transmission, and 50% of all surveyed dentists use protective masks, gloves and eyewear. More than 49% overestimate risks while treating HIV infected patients. More than 55% of all surveyed dentists are willing to treat HIV infected patients. Results of this survey show informational gaps in dentists’ knowledge of HIV infection and highlights the need for additional lectures on HIV infection. |

Abstract: |
A 22 year-old man was referred to our Department for evaluation of white hyperkeratotic multiple flakes, together with tiny erosions localized on both cheeks. Detailed medical history and clinical examination revealed habitual cheek biting as a result of a bad habit, which was manifest with a typical clinical finding. During treatment we applied a combination of salycil acid and corticosteroids as well as vitamin A drops and the patient was advised to stop the bad habit, and to take sedatives during states of nervousness. The therapy resulted in complete regression of the lesions. |

Abstract: |
Stomatopyrosis is a state characterized by a burning sensation in the oral cavity. Various etiological factors contribute to the occurrence of stomatopyrosis. They may be local, systemic and psychogenic. As it was not feasible to gain any accurate and detailed diagnostic and therapeutic effect, and due to the differences in the definition of stomatopyrosis as a disease, we designed a questionnaire which can helpto solve the problem. Our research was conducted on 60 subjects, 30 suffering from stomatopyrosis and 30 in a control group, with no clinical pathological oral finding. Members of both groups were persons of the same age. With regard to sex, both groups included a female population. The testing was conducted by means of a questionnaire, which, apart from general data on the subjects, included subjective and objective assessments of the symptoms present in the oral cavity. General data required referred to the subjects age, sex and occupation. Objective assessment comprised localization of symptoms, their description, intensity of symptoms, their frequency, the time of their occurrence, factors intensifying symptoms, change in taste, etc. Subjective assessment was conducted by means of thermoestesiometric measurement and visual analogue scale (VAS-scale). The findings showed that stomatopyrosis occurs in older subjects, primarily female. The localization of symptoms was confined mostly to lips. Description of the symptoms showed that the burning sensation was the most frequent symptom occurring within the oral cavity. The intensity of the symptoms in the mouth cavity was reported to be unbearable and as far as the frequency of the symptoms is concerned, it was said to be of a continuous nature. The symptoms occur in the daytime, and factors contributing to intensification of the symptoms are anxiety and dentures. The subjects taste was also reported to be changed. Visual analogue scale showed a high subjective assessment of the intensity of the symptoms, while thermoestesiometry did not reveal any inflammatory changes in the oral cavity. These findings suggest that our questionnaire can contribute to better diagnostic and therapeutic effects and help to establish an accurate definition of stomatopyrosis and its occurrence. |

Abstract: |
Candida albicans and other fungi are frequently found in subjects wearing prostheses, especially in prostheses with poor hygiene, i.e. with accumulations of food, plaques and calculi. The aim of this study was to assess the efficacy of Corega extradent relative to fungi adhering to the prosthetic base. Results of the study showed the prosthesis hygiene to be substantially related to inflammation of palatal mucosa. The mean number of fungi per sq.cm of prosthetic base was 64 x 10(5). The number of fungi was redetermined after a two-day treatment with Corega extradent, with unchanged other habits of the prosthesis wearing and cleansing. The number of fungi decreased in all study subjects, the mean value of individual differences being 2238 times. In prostheses with a great number of fungi and extremely poor hygiene, the effect of Corega extradent was poorer, indicating the need of additional mechanical cleansing with a brush. |

As a systemic disease, diabetes mellitus also involves alterations of oral structures as an integral part of the body. A group of 47 diabetics, 34 females and 13 males, mean age 55 years, were included in the study. Mean blood glucose was 12.6 mol/l. Results of clinical studies of oral alterations showed oral symptoms, i.e. xerostomia, to predominate in study subjects, followed by glossopyrosis, stomatopyrosis, gingivitis with hyperkeratosis and exfoliations effecting the tongue and lips. Membranes and ulcerations were less frequent in the group of diabetics under study. Along with the occurrence of metabolic and hormonal disturbances, cardiovascular diseases occupy the first place, followed by locomotor, gastrointestinal, respiratory and renal disturbances. |

Abstract: |
Due to insufficient casual therapy of oral symptoms of dyniae and pyrosis, we applied infrared soft laser in treatment of patients with those oral symptoms. The laser had a wavelength of 904 nm and a radiation strength of 20 W. The tests were performed on 40 persons with stomatopyrosis and stomatodiniae symptoms, under application of laser through 5 days consecutively with radiation of mucosa lasting 3 minutes on 1 cm2. The testing group was represented by persons exclusively under the laser therapy and the control group was represented by 30 persons, who were under a medicamentose therapy with vasodilatator applied with iontophoresis. By persons with stomatopyrosis and stomatodiniae symptoms analgetic effect was attained through the therapy with the laser already after the first application, and the pain and aches intensity was relieved every day during the therapy, by stomatodinae to complete healing. The obtained differences in the intensity of symptoms before and after the therapy were statistically important. The thermoestesiometric determination of laser efficiency by vasodilatation showed differences which were not important, although the temperature during the therapy has slightly increased for 0.1 degrees C. The soft laser can be used as an effective remedy in the treatment of oral symptoms of stomatopyrosis and stomatodinae. |

Abstract: |
As oral lichen ruber is quite frequently encountered in contemporary man, dental physicians should be familiar with the characteristics of the disease. Regardless of various forms of the disease, clinical features of lichen ruber may help in making the diagnosis. Recent research on the pathogenesis of the diseases has pointed to a local immune response. An individual diagnostic and therapeutic approach has been imposed by the complex etiology of the disease. In therapy, antimicrobial agents have resumed their place, along with the use of vitamins. |
