| Abstract: |
The demands of patients for high aesthetics during reconstructive prosthetic procedures accelerated the development of new materials and technology. Forces acting in the post-canine part of the dental arch amount to ≥ 500 N, and thus the material used for fabrication of the restoration must withstand such forces. The aim of this investigation was to study the resistance to fracture of three non-metal materials: 1. ceromer (Targis, Ivoclar-Vivadent), 2. ceromer reinforced with fibre reinforced composite (Targis/Vectris, Ivoclar-Vivadent), and 3. non-metal ceramic system (IPS Empress 2, Ivoclar-Vivadent). Eighteen identical crowns were fabricated from each type of material on a plaster model of a polished natural second lower premolar, according to the manufacturer’s instructions. The same tooth was used for fabrication of a metal model on which crowns were placed in the universal testing device ZWICK. The compressive plate was modified with a pin 7mm in diameter, which acted with force on the occlusal surface at an angle of 180˚. Force was applied up to the first sign of fracture, and the amount recorded.Half of the samples prior to the examination were thermocycled 1000x for 20 seconds at temperatures of 2˚C and 55˚C with 40 second intervals between immersion of the tempered sample. The mean value measured of the amount of force up to fracture for non-thermocycled samples amounted to 577.8 +/- 113.4 N for crowns of ceromer, 923.3+/- 229 N for crowns of ceromer and fibre reinforced composite, and 1208.9 +/- 161.8 N. for crowns of non-metal ceramic. Significant difference was determined by Mann-Whitney U test between all three materials (p < 0.05) for the amounts of loading up to fracture of the samples. Thermocycling did not significantly reduce the amount of force up to fracture of the sample. The investigation determined that all three materials are satisfactory for fabrication of permanent restorations in the area of the masticatory centre, as all withstand force greater than 500 N. |

| Abstract: |
The aim of the investigation was to evaluate the quality of prosthodontic appliances relative to chronic organ system dysfunctions and the level of independency in a group of elderly individuals. The study group included 175 elderly home residents in the Zagreb region over 60 years of age (average 76.8 years, S.D. 10.72), which consisted of 66 male (37%) and 109 female (63%) individuals. The examination included dental status, oral and general health history. Quality of prosthodontic appliances was evaluated according to the CDA index for fixed, and the modified Nevalainen et al. index for removable prosthodontic appliances. Assessment of functional independency of an individual was done using the Activity of Daily Living index by Miura, Araki and Umenai. Over 82% of individuals were in need of fixed, removable or combined prosthodontic treatment. The most common general health conditions involved cardiovascular (29%) and locomotor (18%) systems. Correlation was determined between the poor state of prosthodontic appliances and chronic conditions of CVS (r=0.54, p<0.05) and locomotor (r=0.69, p<0.05) systems, psychological (r=0.88, p<0.05), neurological (r=0.72, p<0.05) and digestive (r=0.65, p<0.05) dysfunctions. Spearman's correlation analysis determined significant correlation between the ADL and the CDA indices scores (r=-0.468, p<0.01), and the ADL and the modified Nevalainen's et al. indices scores (r=-0.572, p<0.05), where lower ADL scores were correlated to the poorer condition of the prosthodontic appliances. |

| Abstract: |
The aim of this study was to evaluate the type and the aesthetic material in relation to age, gender, level of education, employment, socio-economic status and frequency of visits to the dentist. The examination was performed on 212 patients who had a fixed prosthodontic appliance for more than a year (55% males and 45% females, age 18-80 yrs.). The following conclusions were made: 1. The older patients and retired patients had significantly more bridges than crowns (p<0.05). There was no significant difference between gender and patients with a different level of education in the distribution of crowns and bridges (p>0.05). Patients who visit their dentist regularly have significantly more crowns than bridges thanpatients who visit their dentist irregularly or when in pain (p<0.01). Age or the first visit to the dentist made no significant influence on crown and bridge prevalence (p>0.05). 2. Almost all fixed prosthodontic appliances older than 10 years were made of porcelain (98%), while acrylic veneer crowns were more frequent in appliancesolder than 10 or 15 years (p<0.01). Relatively high frequency of porcelain (64%) was recorded in comparison to acrylic material(32%) or chromasite (4%). There was no gender difference with regard to the material used (p>0.05). Patients older than 60 years had more acrylic material compared to younger patients. While patients younger than 39 years had almost exclusively ceramicappliances (p<0.01). Less educated patients had more acrylic veneer appliances. Employed patients had significantly more ceramic appliances than retired patients. |

Loss of a part or complete clinical crown of tooth results in functional and aesthetic impairment of dental arch and complete stomatognathic system. Such teeth, as well as endodontically treated teeth, during reconstructive prosthodontic therapy need to be treated with posts and cores. The aim of the investigation was to measure and compare the dynamic loading resistance of endodonticallytreated teeth with cast metal and prefabricated posts and cores, under dynamic loading fatigue of 800N-1000N, with specific regard to variable root preparation width. The investigation included 48 human lower second premolars of similar dimensions, divided in 4 groupsaccording to type of post and core system (each 6mm long) which they received. 1) prefabricated narrow post and core (Maillefer Switzerland, Size 1) 2) prefabricated wide post and core (Maillefer Switzerland, Size 3B) 3) cast metal narrow post and core 4) cast metal wide post and core. Modified Voss & Meiners dynamic loading tests were performed within a field of 800N-1000N in high frequencypulsator. Teeth with prefabricated narrow posts demonstratedthe highest average dynamic loading resistance of 9.050.622 cycles under 800N until fracture, while teeth with cast metal wide posts demonstrated the lowest average dynamic loading resistance of 3.522.611 cycles under 1000N until fracture. Significant influence of root preparation width on dynamic loading resistance for all typesof posts was determined by ANOVA (p<0.05), with narrow posts demonstrating more fracture resistance. Prefabricated posts and cores demonstrate stable dynamic loading resistance and better intraradicular retention than cast metal posts and cores. |

OBJECTIVES: Mandibular bone height and density were compared with age, sex and the number and type of functional dental units, as represented by Eichner index.MATERIAL AND METHODS: A sample of 150 elderly patients of the Dental Clinic were divided into three age groups, examined and orthopantomograms/OPG/ taken. The OPGs were taken with the copper stepwedge in order to enable computer-assisted calculation ofmandibular optical density and calibration of dimensional measurements by specially designed software.RESULTS:Eichner class III was found in 62% of patients. The presence of functional units was positively correlated with age (r=0.67) and bone height values (r=0.88, p<0.05). Optical density and bone height values of measurements performed at the same locations on mandibles showed no significant correlation (r=0.24,p<0.05). Intra-subject measurement showed that the eminentia piriformis region had significantly higher optical density values compared to 1st molar and mental foramen regions (p<0.05). Women had significantly lower bone height values than men (p<0.05), but no significant sex difference was found in optical density values.CONCLUSION: The presence of functional units has significant influence on bone height, although, like age or sex of the patient, it has no influence on mineral content of the mandible. The eminentia piriformis is not susceptible to bone mineral content or height change. |

Provisional crowns are used as a means of protection for a tooth or teeth on which permanent prosthetic therapy is performed. Their function is multiple: biological, aesthetic and mechanical.The paper presents the case of a female patient with insufficiently filled root canal of the upper middle incisor, and an incorrectly placedand erroneously indicated prefabricated dowel. After extraction of thedowel, three methods are presented for construction of an aestheticcrown on a dowel, serving as a provisional crown during endodonticand permanent prosthetic therapy.The presented methods for construction of provisional work with intraradicular anchorage represent possible guidelines for an aestheticapproach to combined endodontic and prosthetic therapy of frontal teeth. |

Partial or complete deficiency of the clinical crown of non-vital teethleads to a certain degree of deficiency of the dental arch, and to a certain extent disturbed functioning of the masticatory system. In such cases it is necessary to construct posts and cores to strengthen the tooth abutment as the base for crowns and bridges.The aim of the investigation was to compare the resistance of nonvitalteeth, fitted with cast metal and prefabricated posts and cores, todynamic loading in a high frequency pulsator, with special attention todifferent widths of root canal preparation.The results of the fractographic analysis show that the greatest resistance to dynamic loading was found in teeth with narrow prefabricated posts and cores, which endured loading of 1000N for an average 7,497,318 cycles, without the occurrence of fracture. Widening of the root canal in the case of preparation of the intraradicular part of the tooth for posts and cores, of more than 3.5 mm, significantly reduced its retention and increased the possibility of root fracture. All fractures occurred in the tooth root, while the posts and cores retained intact. |

A temporary crown or bridge is the best protection of a prepared tooth from pain, saliva contamination and caries propagation. It must have all the properties and qualities of a permanent restoration in order to properly function in the oral cavity.The aim of the investigation was to determine the influence of two types of cervical preparation of the abutment, as well as the influence of cyclic thermal changes (5°C-55°C), on the dynamic strength of temporary crowns made of poly(methilmethacrylate) resin. The fatigue tests were performed in an Amsler's high frequency pulzator under cyclic dynamic loading in the range from 400N to 1000N.The results of the investigation showed negative reciprocal relationship between the values of the dynamic load and the number of cycles endured without the fracture of the sample (r=-0.98). The thermocycled samples demonstrated significantly lower fracture resistance than the nonthermocycled samples (p>0.05). |

The entire osseous system of transmission of mastication forces may be divided into five smaller units: the lower jaw, the anterior and posterior system of facial mid-portion, the frontal bone with the base of the cerebral cranial segment and the cerebral part of the skull. The development of these regions is simultaneously influenced by genetic determinants and numerous external (environment, climate) and general factors (statics, gravity, upright walking).The objective was to study basic craniometric and skeletotopic characteristics of the facial skeleton and hard palate in osteologic samples kept at the "Drago PeroviE" Department of Anatomy. A total of 64 skulls pertaining to both sexes were included in the study. Following determination of craniometric points, measurements of twelve selected indicators were made by electron gliding meter with 0.01 error probability and by goniometer with 1 percent error probability. The obtained results were compared with the data of similar studies and, together with other harmonic distances, they were mathematically inserted into a harmonic circle with the diameter of 2R. The height of the skull is taken to be 2R, which is one sixth of an adult person's height.The obtained measurements have ensured the basis for further mathematical and structural analyses of the cranial structure. |

| Abstract: |
Restoration with post and cores for fractured clinical crowns on devitalized teeth is the treatment procedure frequently used in the clinical practice of specialists and dentists in primary health care. The paper deals with prosthetic reasons for teeth devitalization and making of post and cores, particularly when a comparatively large rests of the clinical crown are preserved, along with the indications and contraindications for their placement on devitalized teeth.Influence of biomechanical factors is analyzed with regard to post and core retention and stress distribution over the hard dental tissue and surroundings, depending on possible variations in preparation width and length, particularly in the radicular part, for different types of post and cores.In conclusion, the authors express their preference for individual post and core placement over prefabricated, and highlight the importance of correctly determined indication for post and core, successful selection of the type of post and core and correct treatment for extension of the lifetime of the fixed prosthetic work on the endodontically treated tooth. |

| Abstract: |
Average amounts of palladium-silver alloy were assessed in a sample of 966 veneer crowns and 667 casted posts. The mean weight values of the samples under study were compared in relation to particular types of anterior teeth. The specimens cast in Auropal SE were weight by the precise balance, Tehtnica, type 6215. Results were analyzed by weight according to the type of the sample under study, and classified according to age, and to the anterior teeth of the upper and lower jaws. On an average, the heaviest posts and veneer crowns were measured on upper canines, whereas the lighest were those on lower incisors. A cast post was found to be by about 40% heavier than a veneer crown on the same tooth. |
