| Abstract: |
Prosthetic solutions on implants are frequent treatment plans that patients accept well. In some states there are dental implant registries that are used as databases for patient information and for different professional and administrative procedures. The connection between a natural tooth and an implant is still a matter of debate, and the reason is the ankylotic connection at the implant versus periodontal ligament that the tooth possesses. These two cases show successful prosthetic reconstruction and patient satisfaction. |

Bisphosphonates, drugs that influence bone structure, are used in treatment of a number of bone diseases, from osteoporosis to malignomas with subsequent osteolysis. Recently bisphosphonates have been linked to jaw osteonecrosis. These changes are more frequent in patients receiving parenteral bisphosphonates (zolendronate and pamidronate), while in others, such as alendronate, they are not so frequent. It has been shown that the duration of therapy increases the risk of jaw osteonecrosis.Osteonecrosis of the jaw, as susceptible regions, is explained by the fact that the bone is under constant stress during chewing, as well as under constant invasion with oral microorganisms. It has been suggested that in treating of jaw osteonecrosis one should combine antibiotics with oral surgical procedures. Since the knowledge on this correlation is limited, it has been recommended that patients with increased risk of osteonecrosis should perform preventive measures such as good oral hygiene, adaptation of removable prosthetic devices, restauration of carious lesions and endodontic treatment. Dental examination is necessary before commencing bisphosphonate therapy. We present a case of an elderly male patient with advanced osteonecrosis and subsequent vertebral compression fractures. |

| Abstract: |
Implantoprosthetic therapy of the maxillary arch can be performed in various ways. Clinical and radiographic findings help us to find the right selection and treatment planning. This presentation show one of the possible solutions for complete edentoulism of the maxilla. We performed bilateral sinus lift as preparation for placement of six implants and fabrication of a fixed prosthetic construction. We can conclude that this type of treatment gives excellent esthetic and functional results. |

Today, when dental implants are routinely used, the problem of single tooth replacement is often resolved with implantoprosthetic treatment. Modern medicine prefers the use of body’s own tissues and organs. During seventies in the 20th century tooth follicles were often used for single tooth replacement, but with the ascendance of implants they seem to be forgotten. This presentation displays current trends in transplantation medicine of tooth follicles, showing a case of lower first molar replacement with a follicle of a third molar. |

| Abstract: |
Condensing osteitis is pathologic growth of the maxillomandibular bones, characterized by mild clinical symptoms. Bone thickening refl ects impaired bone rearrangement in response to the mild infection of dental pulp. This case report describes a patient with extreme bone thickening localized in the left maxilla causing facial asymmetry. Clinical and radiological examination failed to point to defi nitive diagnosis. On differential diagnosis, all bone tissue tumors, cementoma and condensing osteitis with prolonged unsuccessful conservative endodontic treatment of the upper fi rst molar, were considered. A surgical procedure in local anesthesia was preformed in order to remove the pathologically altered bone mass and to extract the tooth that could not have been treated successfully. Histopathologic analysis confi rmed the infl ammatory etiology of the bone lesion. Clinical and radiological follow-up indicated complete healing of the area, with appropriate density and trabeculation of the bone. This case presents as an extreme example of condensing osteitis with major bone lesion, caused by a mild tooth infection. |

In everyday dental practice the problem of prosthetic solution of total edentulousness in a more or less marked atrophic lower jaw, is a particularly difficult problem. Beside classical prior prosthetic procedures such problems can also be solved with the placement of dental implants. It is then essential to carefully plan the position of the implant, calculate height, width and density of the bone on the planned area, calculate anatomic and intermaxillary relations and plan the correct superstructure in order to ensure for the patient optimal retention and stabilisation of the prosthetic restoration. Such a restoration must functionally, aesthetically and phonetically satisfy the patient. Whether the condition of the lower edentulousness with implantoprosthetic rehabilitation is solved with a mobile prosthetic or fixed prosthetic superstructure depends on the possibilities of inserting a particular number of implants, the patient’s wish and also financial possibilities. The paper presents the case of a 58-year-old female patient who came to the surgery because of loose fixed prosthetic devices in the lateral region of periodontologically compromised frontal lower jaw. The patient had a total denture in the upper jaw with which she was satisfied. After radiographic treatment extraction of the remaining teeth was indicated and implantoprosthetic rehabilitation agreed. With the healing of the extractive wounds, a further orthopantomogram showed the possibility of inserting only four Ankylos implants in regions 46, 43, 33, 36. The patient expressed the desire to again have fixed prosthetic rehabilitation. As a compromise, at the end of osseointegration, the implants were loaded with a removable bridge on the telescope principle. The paper presents one of the methods of implantoprosthetic rehabilitation of the atrophic lower jaw. |

The increasing use of dental implants in everyday dental practice, and the systematic and documented years of monitoring patients with implanted and prosthetically supplied dental implants, leads to frequent confrontations of the therapist with different kinds of complications, and also failures. Although complications and failures are not the same, they are closely connected in dental implantology. However, not every complication necessarily leads to failure. Thus every therapist must be capable of envisaging possible complications, and eventually prevent them in time, or if they arise to know how to treat them. Complications in dental implantology can be divided into surgical and prosthetic complications. Surgical complications can be intraoperative, early postoperative and late postoperative complications, of which the most frequent are periimplant changes/lesions. Prosthetic complications are usually connected with unsatisfactory direction and localisation of the implant, instability and fracture of elements of the reinforcement, fracture of bridge constructions, aesthetic and functional complications and loss of the implant. In the broadest sense failure in dental implantology is clearly loss of the implant due to any reason, but also dissatisfaction of the patient with the functional or aesthetic effect also leads to failure. The paper presents our experience to date with complications, and also failures, during the use of different types of implantological systems. |

Surgery of head and neck tumours is complex and unequivocally expensive. There have been very few investigations to date in the world on the cost of treatment of this group of patients, and in Croatia not one. The aim of the present study was to determine the cost of treating patients with head and neck tumours, its structure and factors which have a significant influence on it. The study included 248 patients, treated in the Clinic of Maxillofacial Surgery in Zagreb during the period 1995 - 1996. The cost of treatment is presented as the total cost of hospital expenses, diagnostics, operations, anaesthesia and radiotherapy, and its dependence on all factors tested which we assumed to have a significant effect on it.The average cost of treatment for the patient with head and neck tumours is 17,297 kn. The duration of hospitalisation, type of ablation and reconstructive surgical procedure has the greatest influence on the cost of treatment. Thus, intervention in this segment would enable the greatest savings. As the duration of treatment and type of therapy is decided by the physician, the planning and realisation of cheaper treatment is the responsibility of the physician. |

According to data of the World Health Organisation, malignant diseases, apart from cardiovascular diseases, are the most prevalent diseases of today.As one form of therapy for malignancy radiation in 63% of patients has the primary task of destroying tumour cells, while minimally damaging the surrounding tissue.The greatest and most dangerous complication in radiotherapy of the head and neck is without doubt osteoradionecrosis (ORN). It occurs most frequently when the dose of radiation is more than 60 Gy or in the case of patients who receive combined radio and chemo-therapy. ORN occurs in 5-22% of such irradiated patients. ORN can occur spontaneously, although in 60% of cases it occurs as a response to tissue injury, usually after tooth extraction, but also after other manipulations in the oral cavity. Because of the radiation the bone becomes acellular, avascular and hypoxic, and clinically can be interpreted as the occurrence of ulceration, mucous membrane necrosis and exposure of necrotic bone, with pain and eventual paresthesia. Predilective sites are the posterior parts of the mandible. For diagnosis of ORN, apart from a medical examination, orthopantomogram, computerised tomography and magnetic resonance are needed. In the future SPECT (single-photon emission computed tomography) will have an important role.Clinical indicators in the therapy of ORN are first and foremost a good preventive programme and periradiational care of the patient undergoing radiation, and in the case that changes do occur surgical therapy is needed, therapy with hydrobaric oxygen (HBO), and the use of Marx's protocol. Presentation of numerous examples from our practice shows how many clinical indicators are implemented. |

The paper presents the oral rehabilitation of a patient with restricted opening of the mouth, caused by a combat injury of the soft and hard tissues of the maxilla, with the object of achieving satisfactory aesthetics, sufficient loading of the abutment teeth and, finally, finding the most suitable course for introducing a mobile-prosthetic restoration, and at the same time avoiding damage to soft facial structures.The patient was supplied with upper partial dentures with a reduced metal base, retained with conus crowns and a lower full denture with the possibility of its later adaptation into a mandibular overdenture retained by implants. |

The forces of muscular exertion which develop during daily physical exercise stimulate the process of bone formation in the areas of the insertions of the exerted muscles, resulting in the strengthening of bone, in terms of increased mineral component of the bone and increase in its density.The object of this study was to assess the density of mandibular bones in a group of active rowers, compare the measured values with values obtained in a control group and to examine whether there is statistically significant difference in the total sample, depending on the different body mass index of the subjects.Eight active rowers took part in the study and seven subjects in a control group. An orthopantomogram was performed in all subjects and the values of density measured on them were expressed in equivalent thickness of the copper calibration pin.In all points measured from the right and left sides of the angulus mandible mandibular density in rowers was higher than the same in the control group. Furthermore, in all points measured from the right and left side of the angulus mandible mandibular density in the group with higher values of body mass index was higher than the same in the group with lower values of body mass index. No statistically significant difference was recorded (p>0.05).Increased physical activity inevitably leads to thickening of bone structure in the areas of the insertions of the exerted group of muscles and in order to compensate for the effect of the forces of exertion on surrounding tissue. |

Loss of a large number of teeth in the lateral segment of the maxilla, either on one side or both, represents a challenging therapeutic procedure. Today the demands of the patient have increased with regard to fixed prosthetic treatment for lateral segments of edentia.Although lateral edentia can more easily be solved by a partial prosthesis or a cantilever bridge, dental implantology is now one of the solutions in the daily practice of the dentist. One of the most frequent problems in solving distal edentia of the maxilla by the use of dental implants is connected with the anatomic relations between the maxillary sinus and alveolar prolongation of the maxilla, i.e. the socalled deep recess of the maxillary sinus, or even the result of severe atrophy of the alveolar prolongation. Such situations do not facilitate insertion and good stabilisation of dental implants, and thus they narrow indications for fixed prosthetic work. With the development of dental implantology during the 1980s, a possible solution for this problem arose, by an operation to raise the floor of the sinus, which today is known as “sinuslifting”. Prospectively, operation techniques are diverse, as are also the different materials used in these operations. Sinuslifting is in fact a reconstructive surgical procedure, which with osteal implants or osteal restorations achieves the formation of bone on the floor of the maxillary sinus in the alveolar recess, where the dental implant will later be inserted. Today's technique is briefly described as the opening of the lateral osteal wall of the maxillary sinus, and this osseous fragment is twisted towards the interior of the sinus cavity and the resulting area (caudal third) filled with augmentation material. If there is sufficient height of the bone in the area of the floor of the sinus (4-5 mm) the implant can be carried out at the same time as sinuslifting, to ensure safe primary stabilisation of the dental implant.We present the case of a 33-year old patient with sinuslifting in the right segment of the maxilla, as preparation for dental implant. The patient already has osseointegrated implants in area 46 and 37. |

Definition: Non-Hodgkin lymphomas (NHL) are a heterogenetic group of lymphoproliferative neoplasms characterised by the occurrence of malignantly changed lymphocytes in a lymph node, and rarely primary in other organs.The heterogenicity of the disease is interpreted in the diverse clinical manifestations of the disease, diverse laboratory parameters, histological finding, immunological origin of the malignant cells, reaction to therapy and disease prognosis. The disease is most frequently interpreted by painless enlarged lymph nodes, and more rarely with disturbances in other organic systems due to lymphomic infiltration of organs. Diffuse giant cell NHL B-immunophenotype belongs to the group of lymphomas of moderate degree of malignancy. It occurs in middle-aged and older age groups, usually with disseminated lymph nodes, and up to 70% of patients have infiltration of the bone marrow by lymphoid cells. Diffuse lymphomas of giant cells are very often localised and infiltration of the gastrointestinal tract and central nervous system is a frequent occurrence.In this study we describe the method of diagnosing diffuse giant cell NHL B-immunophenotype, which was detected after inadequate healing of postextraction alveola.The patient was admitted because of a swelling and growth from the post-extraction alveola in the area of the lower left wisdom tooth and anaesthesia of the chin. The tooth was extracted because of pain in the tooth and gingiva. The patient informed us that approximately 6 months ago he had felt “tingling” on the left side of the chin. At that time he was treated with laser therapy by a specialist in oral medicine, after which partial improvement occurred. However anaesthesia of the area, pain and swelling recurred, and it was decided to perform extraction of the lower wisdom tooth. The patient was treated in this Department initially with cochleation and antibiotics. Improvement occurred one week after treatment. However, after the remission pain and swelling recurred. A detailed history was taken and it was learnt that a few months beforehand, during an operation for an inguinal hernia, a problem had occurred with coagulation, and renal enzymes were raised and hepatitis suspected. A biopsy was then taken in our Department and sent for PHD. Palpable lymph nodes were not found in the area of the head and neck. During analysis of the first biopsy the result was suspect for malignant lymphoma and the patient was sent for further treatment by a haematopathologist, where a further biopsy was taken. The finding showed diffuse giant-cell Non-Hodgkin lymphoma B-immunophenotype. The patient is presently undergoing treatment in the Clinical Department of Haematology, and after appropriate therapy the disease is in remission. |

Non-Hodgkin lymphomas (NHL) are a heterogenetic group of lymphoproliferative neoplasms, characterised by the occurrence of malignantly changed lymphocytes in a lymph node, and rarely primary in other organs. The disease most frequently manifests with painlessly enlarged lymph nodes, which in the gastrointestinal and urinary tract may cause compression. Infiltration of the bone marrow causes insufficiency of myelopoiesis, resulting in anaemia, trombocytopenia and neutropenia. Classification of the disease is based on PHD and peripheral blood findings.Extranodular disease occurs most frequently in the gastrointestinal tract, and also often occurs in the sinuses, thyroid gland, salivary glands, breasts, kidneys and lungs. Approximately 25% of extranodular lymphomas first occur on the head and neck in the so called Waldeyer's ring, although primary lymphomas of the oral cavity are rare.We present the case of a 63-year-old man with primary occurrence of NHL on the gingiva. The suspect lesion in the oral cavity was noticed during extraction of an inflamed root in the upper jaw. The lesion was located in the immediate vicinity of odontogenic inflammation, although it was not connected with it, and thus a specimen was taken for biopsy. The radiographic finding did not show anything specific. PHD finding of the lesion showed necrosis, bleeding and polymorphonuclear inflamed infiltrate and a tumorous mass with atypical lymphocyte cells. After a further examination by a haematopathologist NHL was confirmed with a high degree of B-cell malignancy. Immunohistochemistry confirmed giant cell NHL, B immunophenotype, and the patient's condition was defined as NHL II E. |

The study presents the uncommon clinical appearance and localisation of carcinoma of the upper lip.A 74-year-old female patient was referred to the Outpatient Department of Oral Medicine because of a purulent infiltrate in the upper lip which was thought to be an insect bite. She was referred to the Outpatient Department of Oral Surgery where an incision was performed and drainage, and piogenic granuloma diagnosed. As the lesion had not healed after two months, but on the contrary had become harder and more visible, both vestibularly and labially, an excision was planned. In the meantime, however, the patient had attended the Clinic of Dermatovenerology because of basocellular lesions on the face, and during excision of the facial lesions a surface biopsy was performed from the skin side of the lip, and the PHD finding showed planocellular carcinoma. At the Clinic of Dermatovenerology the patient was recommended to undergo radiotherapy of the lesions. However, the patient returned to the Out-patient Department of Oral Surgery. Because of the suspicion that the biopsy had been too superficial and because of the possibility of eventual confusion with a mioblastoma, complete excision of the lesion was performed and an identical PHD finding was obtained of planocellular carcinoma. The patient was then transferred to the Department of Maxillofacial Surgery, University Hospital Dubrava, where an extended excision was performed, and the PHD confirmed that the lesion had been completely excised in the previous procedure. |

We present the case of unusual metastasis of a facial melanoma in a lymph node of the neck, after sixteen years.A 40-year-old male patient was referred to the oral surgeon because of an enlarged lymph node on the right side, region one. Due to the fact that he had a number of untreated teeth and radiating pain on the right side of the face he was admitted for examination with suspected odontogenic lymphadenitis. From the case history it was learnt that the patient had had an operation for melanoma in the right nasolabial cheek sixteen years previously. An examination revealed a postoperative scar on the right cheek, which was hard and painless, and a lymph node, two centimetres in size in region one, and also several untreated teeth. The patient was first sent for cytopuncture of the node, which showed metastasis of the melanoma. For preoperative preparation radical dissection was performed of the right side of the neck and all teeth extracted which could not be treated. Only one positive lymph node was found In the PHD specimen. The patient was included in the protocol for melanoma and all tests were carried out according to clinical guidelines.As all findings are presently normal no further therapy is necessary.The case shows that even in such situations we cannot disregard enlarged lymph nodes of the neck in differential diagnostics. |

The object of the study was to determine the size of the mandibular angle, height of the ramus and length of the mandibular corpus, and to compare the obtained results, depending on the type of skeletal orthodontic anomaly, age and gender of the subject. Seventy-seven subjects participated in the study (30 male and 47 female) with diagnosed anomalies of open bite, mandibular prognathism and Class II/2 malocclusion, classified into three age groups. The following variables were measured on a laterolateral cephalometric radiograph: mandibular angle, height of the ramus and length of the mandibular corpus. The mandibular angle was smallest in the subjects with over bite (average 123°), largest in patients with open bite (average 135°) and in subjects with progeny it amounted to an average 130°. Statistically significant difference was found between the size of the mandibular angle and the type of anomaly (p<0.05). Statistically significant difference was determined between linear skeletal variables and age groups (p<0.05). With regard to the values of the height of the ramus and length of the mandibular corpus statistically significant difference was found between the first age group (6-12 years) and the third age group (16-18 years) (p<0.05). In the male subjects values for linear skeletal variables were higher than the same variables measured in female subjects, although the difference was not statistically significant. |

Pain is known to be a frequent reason for the patient’s arrival to the dental clinic. The aim of this study was to determine how often patients come to the dental clinic because of toothache or pain in the oral cavity; how high the percentage of acute or chronic pain, and the distribution of the aforementioned pains according to the sex of the patients.Examination of the frequency of pain in dental patients was carried out on a sample of 2,735 subjects during a period of approximately one year. Pain was determined in 16.49% of patients and the remaining 83.51% of patients were without pain. There were slightly more female patients (54%) than male (46%), although correlation between pain and sex of the patients was not statistically significant. The higher number of female patients was the result of the greater number of female patients admitted. With regard to the duration of pain, acute pain was significantly more frequent (around 84%) than chronic pain (around 16%). Within the structure of acute and chronic pain odontalgia was most frequent in 89% of all the acute pains and in 90% of all chronic pains, which was significantly more than all other types of pain. The frequency of acute odontalgia, with respect to the total number of admitted patients, amounted to 12.36% of patients and chronic odontalgia 2.38% of patients. |

The risk of malignant transformation of oral lichen planus remains a controversial issue in the literature. Chronicity of oral lesions has been shown to present low risk of oral cancer which has been identified as arising from areas of erythematous atrophic and erosive lichen planus. It is estimated that malignant transformation of OLP occurs in 0.4 - 2.5% of cases, mostly those with lichenoid dysplasia. Recent molecular genetic studies showed minimal genetic deviation in lesions of oral lichen planus, while the epithelial dysplasia and alignant lesions have shown increased genetic alterations.We report two well-documented cases of long-standing oral lichen planus in which squamous cell carcinoma of the buccal mucosa occurred.Presented cases document and warn of the propensity of oral lichen planus to undergo malignant transformation and stresses the importance of regular histilogical follow up; biopsy at the initial visit to confirm the diagnosis and more frequent control biopsies during clinical follow up to assess histological changes that may represent dysplasia. As a lesion at risk malignant transformation, OLP also needs to be confirmed by use of genetic markers. |

The healing of bone defects with or without reconstruction today is relatively satisfactory. Large bone defects of the jaw, so far, have been filled with various types of bone implants (autologous, homologous, xenogenic and alloplastic).The best implant will be after implantation in bone defects, would stimulate the formation of bone, form and firm conection with the base and have the characteristics of bone.The investigation included two patients with large bone defects after removal of jaw cysts. To fill the bone defects we used bone substitute Chron OSTM .Chron OSTM is syntetic ß tricalcium phosphate. It is an alloplastic bone implant. Previous investigations showed that Chron OSTM is osteoconductive, resorbable and highly biocompatible with compressive strength similar to the compressive strength of cancellous bone.Results are presented on the basis of clinical followup and X-ray performed preoperativly, postoperativly and after six months.The patients were screened using panoramic X-rays standardised with copper stepwedge. Using microdensitometric method all the measured values were expressed in the equivalents of actual copper stepwedge thickness. |

Dens invaginatus is a developmental variation thought to arise as a result of invainatio in the surface of the tooth crown before calcification has occurred. The authors described two cases of dens invaginatus accompanied by different periapical complications in children, as wel as the therapy methods used. The two children were between the ages of 12 and 16 years. The type of dens invaginatus was classified according to Schulze and Brand.Both cases had dens invaginatus on their permanent teeth in the maxilla. The complications occurred while the teeth were growing, and they were accompanied by swelling in the region of the dens invaginatus. In Case 1, pulp extripation was accompanied by profuse pulp chamber bleeding. Intraradicular instrumentation was adapted to conform with the existent morphological deviation of the endodontic space. The root canal was temporally filled with calcium hydroxide for a period of 10 days. The pain disappeared on the first day and the swelling gradually reduced. On the tenth day, the temporary root filling was replaced, and the root canals were dilled using the lateral condensation with standardized gutta percha points and paste. In Case 2, the clinical examination found fluctuation in the vestibular region. An intraoral incision with drainage was performed and antibiotic treatment continued. When the odontogenic infection has been controlled, the dens invaginatus tooth was surgically extracted.Pathohistological finding of the periradicular cyst confirmed the diagnosis of a follicular cyst. Given the significant size of the post-surgical bone defect, it required corresponding treatment with Bio-Oss spongiosa block and membrane for GBR.The periapical complications required early diagnostic and endodontic treatment to prevent further difficulties at a later stage. |

Electrochemical conditions in the oral cavity lead to a release of metal ions into the patient's saliva. The aim of this study was to examine and compare the types andquantities of metal ions released from two base alloys: Co-Cr-Mo alloy (WironitR, Bego, Germany) and Ni-Cr alloy (Wiron 99, Bego, Germany) under in vitro conditionsimitating artificial saliva. We soaked ten sets of each alloy having 497 mm2 exposure surface for 1, 2, 3, 4, 5, 6, 7, 14, 21 and 30 days (six pieces each set) in phosphate buffered saline (pH 6.0). TE in the phosphate buffered saline (saliva) were assessed by ICP-AES (JY50P, Jobin-Ywon, France) with the detection limit of 10 mg/L. We found detectable amounts (mg/L) of TE (Mean SD) released from Co-Cr-Mo alloy (Mean SD): Co 337 (170), Fe 21 (15) Zn 87 (56), Ni 41 (68), and Cr 49 (42) and detectable amounts of TE released from Ni-Cr alloy (Mean SD): Co 265 (300), Fe 247 (256) Zn 92 (46), Ni542 (668), and Cr 396 (410). The manufacturer did not indicate the presence of Fe, Zn, and Ni in the Co-Cr-Mo alloy and the presence of Fe, Co and Zn, in the Ni-Cr alloy. A significantly higher amount of Fe, Ni and Cr was released from Ni-Cr alloy (p<0.05), and a considerablyhigher amount of Co was released from Co-Cr-Mo alloy, although it did not reach a statistically significant level (p>0.05), while there was no significant difference between the two alloys for Fe ion release (p>0.05). We must keep in mind that the amount of the released TE may be much higher than the reported values in this study, afterthe laboratory procedures (casting, polishing, etc.) and allergenic essential TE Cr, Co, and Ni may be present locally in a considerably higher amount. |

When planning retention one of the most important tasks is to ensure that the construction, size and form of the retentive elements enable the transfer of forces which the loaded tissues can accept without damage. The use of magnets fabricated from biocompatible materials is becoming increasingly used as a means of retention for total and partial prosthetic replacements and resectional prostheses. The purpose of this study was to examine factors on which the strength of retentive forces Sm-Co5 and Nd-Fe-B magnets depend. The samples were divided into two groups: 28 pairs of Sm-Co5 magnets and 28 pairs of Nd-Fe-B magnets. The heights of the measured magnets were 2, 2.5 and 3 mm. Each sample consisted of a group with lateral force and a group without lateral force. The force values were measured by means of a device for measuring magnetic retentive forces, which enables the measurement of retentive forces of different combinations of permanent magnets and ferromagnetic materials. The results are presented in figures and tables. The basic factors that influence the magnitude of the retentive magnetic force are the type of magnet, size, their mutual distance and the presence of lateral forces. The values obtained indicate which sizes and thickness of the magnetic pairs should be used for the desired force of retention. |

The purpose of the study was to determine the prevalence of clinical signs and symptoms of temporomandibular disorders (TMD) in a young adult non-patient population and to investigate the possibility of an association between TMD and occlusal factors. A questionnaire including data from history and clinical functional examination was used in the study. All subjects (a total of 230) were male (army recruits), from 19 to 28 years of age (mean 21.3 ± 2.1). The study indicated that 38% of subjects had at least one symptom (subjects with slight, moderate and severe discomfort), while 45% of subjects had at least one sign of TMD. Spearman's rank correlation test showed weak but statistically significant correlation (p < 0.05 and p < 0.01) between occlusal factors and TMD in a young adult non-patient population. Clinical signs and symptoms of TMD were weakly correlated with some occlusal factors (malocclusion traits (Angle classes II/1, II/2, III, and crossbite), slide between RCP and ICP ≥ 1 mm, midline discrepancy ≥ 2 mm, nonworking side interferences, horizontal overlap ≥ 5 mm) and parafunctional habits (teeth clenching and teeth grinding). However, the importance of association between TMD, occlusal factors and parafunctional habits should not be overstated (weak correlation), since this may lead to neglect of the many other causes of orofacial pain and dysfunction in a biologically multifactorial system. |

The aim of this study was to determine correlation between the width of upper incisors (WUI) or the width between the tips of the upper canines (WTC), or the width between distoaproximal surfaces of upper canines (WDaC) with the width of the alae of the nose (WAN) or the width between the corners of the mouth (WCM) on a representative sample. The aim was also to calculate the ratio between the measured variables, which could be helpful in clinical practice.For this purpose two thousand individuals aged from 18 to 24 years, with intact frontal teeth were measured. There were 920 males and 1080 females.According to statistical analysis of the results it was concluded: 1. Statistically significant difference between males and females existed for all the measured variables (WAN, WCM, WUI, WTC and WDaC), i.e. males had bigger dimensions. 2.There was no significant difference between sexes for all the calculated ratios: WCM/WAN, WAN/WUI, WCM/WDaC, WAN/WTC, WDaC/WAN (p>0.05). 3. The width of the nose approximates to the width between the tips of the cusps of upper canines (1.08:1). The other calculated ratios are: WAN/WUI = 1.206:1; WCM:WDaC = 1.228:1, WDac:WAN = 1.158:1 and these ratios together with the measured mean values: WUI, WCT, WDaC in the examined population, both for men and women, could be helpful for the choice of the proper size of upper frontal artificial teeth and for their setting in the wax of the denture. |

The problem of healing bone defects of odontogenetic aetiology is the most frequent cause of failure in surgical treatment of an ostitic process. The aim of this study was to valorise the successfulness of healing of bone defects after implantation of a new alloplastic copolymer - polyglycol bone implant (Fisiograft). A group of 45 subjects was examined with an ostitic process on teeth of the intracanine region. The results were valorised on the basis of densitometric measurement over a period of 12 months after implantation. The results obtained indicate that polyglycol copolymer bone implants can be successfully used in the treatment of bone defects of odontogenetic aetiology. Their fundamental advantage is slower biodegradation, which ensures a more suitable area for the apposition of new bone in the lumen of the bone defect, simple application in clinical work and the possibility of a mutual combination of all three available forms. |

The aim of this study was to evaluate patients' satisfaction with their partial dentures in relation to some socio-economic variables. The aim was also to evaluate patients' satisfaction with denture retention, speech, aesthetics, chewing ability and comfort while wearing dentures. The influence of some factors, such as age, sex, marital status, level of education, smoking, chronic diseases, socio-economic status, ordinal number and age of thepresent dentures, on patients' satisfaction with partial dentures was also evaluated. A total of 165 patients with partial dentures took part in the study. There were 59 males and 106 females aged between 38 and 87 years. Patients graded their satisfaction with their partial dentures in total and then graded aesthetics, speech and retention, chewing ability and comfort while wearing dentures by using a scale from 1 to 5. Following a statistical analysis the following conclusions were made: 1. Patients were mostly satisfied with their partial dentures (the distribution of the scores of the patients' assessments was asymmetrical towards the highest scores in all examined categories). Only 3.2% of the patients were not absolutely satisfied with their partial dentures, 2. More then half of the examined patients scored all the examined variables in the best score category (5), 3. Patients of a higher level of education had higher criteria for the aesthetic appearance, as they assessed their partial denture aesthetics with a lower grades than patients with lower level of education. Males were less satisfied with the chewing ability of lower partial dentures (p<0.05) than females, 4. There was no significant difference in patients' assessments of the quality of their partial dentures between different age groups, ability of self-supporting life, socio-economic and economic status, marital status, smoking, presence of chronic diseases, number of previous dentures and age of present dentures (p>0.05). |

The object of the study was to investigate where and in what way iconography and homage to St. Appolonia, patron saint of the dental profession, can be found in the Franciscan province of St. Cyril and Methodius.The study included 30 monasteries and counties and data were collected by means of a questionnaire, electronic communication and the exhibition "Peace and Goodwill" in Zagreb. The material was analysed in tables and by photo-documentation. Nine altar statues and one altar painting of St. Appolonia are situated in 9 (30%) of the Franciscan churches investigated. The churches are mainly situated in north-west Croatia, which is a result of the socio-political and organisational influence of Central Europe in these areas. Iconographically the figures of St. Appolonia conform to the distinctive iconography of the Roman Catholic Church and include an element of local influence. The sculptures are made in wood, polychromed and gold plated. They range in height from 100 to 184 cm. A painting, oil on canvas, measures 254 x 142 cm. All the figures of St. Appolonia are on altars, indicating that the saint is very highly revered and homage to her is permanently firmly established in the spiritual tradition of the Franciscan Order. |

Bone morphogenetic proteins are a group of nine known proteins which represent factors of growth and differentiation with the ability of inducing new bone. Osteoinduction is a precisely defined sequence of biological reactions which lead to the transformation of mesenchymal cells into cartilage and bone.In vitro and in vivo, studies have demonstrated that in order for proteins to realise their clinical function they need carriers. So far ACS (absorbable collagen sponge) has been most frequently used as a carrier. However the search for the best carrier continues. Investigations carried out so far on experimental models (rats, dogs, minipigs and chimpanzees), and in human volunteers, have indicated the great potential of BMP in the reconstruction of bone defects of the stomatognathic area. Promising results have been obtained so far in periodontal surgery, augmentation of alveolar ridges, augmentation of the floor of the maxillary sinus, treatment of periimplantitis and treatment of larger bone defects after extirpation of tumours. |

Large bone defects of the jaw have, so far, been filled with various types of bone implants, mainly synthetic nonresporative aloplastic implants (hydroxilapatite). As we have data only for two types of resorptive xenogenic bone implants, the purpose of this investigation was to examine resorption and osteoinductive capability of OSTEOVIT as a material for filling bone defects after removing large cysts (more than 20 mm in diameter) and after apicectomy of the tooth, where more than 1/3 of the tooth has no strong bone bases.Osteovit is a collagen matrix of calf spongiose consisting of porous collagen purified of antigens, fats minerals, enzymes and all other noncollagen materials.Forty-one patients, 9 female and 32 male aged 15-54 years were included in the investigation Nineteen of them had a clinical diagnosis of OPC and 22 a clinical diagnosis of cysts radicularis. All patients had a indication for apicectomy indicating that they had bone defects larger than 20 mm in diameter or destructive alveolus along more than 1/3 of the tooth root.The results are shown on the basis of a clinical follow up and radiographic examination 6 months, 1,2,3 and 4 years after surgery. If, in some cases, a fistula was found during the postoperative followup the treatment was recorded as clinically unsuccessful. Success on the basis of radiographic criteria was also evaluated. First we classified defects into periapical (circumscriptive) and those along the tooth root and than divided the rays into two groups.Unfortunately results for only for 22 patients were abtained and the others were excluded from the investigation because of incomplete follow-up.With regard to total success, according to clinical criterium, 20/22 patients had good clinical diagnosis, i.e. 91%. By radiographic analysis we found complete healing in 13/22 patients or 59%. We also analysed results according to the localization of the periapical change. It was concluded that patients with circumscriptive changes were 88% clinically successful while those with a defect along the tooth root were 93% clinically successful.According to the results of this investigation it can be concluded that OSTEOVIT is a very good resorptive material for filling bone defects and with OSTEOVIT it is possible to widen indication for apicotomy to include cases where more than 1/3 of the tooth has no strong bone base. |

| Abstract: |
The paper describes the unusual localisation of a granular myoblastoma. Granular myoblastoma, or myoblastoma of granular cells, is a tumour of controversial etiology, and according to its rare occurrence in tissues, is an entity in itself. It occurs most frequently in subcutaneous tissues, skin, the breasts, skeletal muscles, and also intraoral tissues, where it occurs most often in the tongue. In the presented case the tumour occurred on the upper lip, on the border of the lip redness, and after excisional biopsy granular cell myoblastoma was confirmed. According to all criteria the tumour histologically answered the description of the described tumour, and as it was also partially found subcutaneously, pseudoepitheliomatous hyperplasia of the epithelia had occurred. As the alteration was completely excised no further treatment was necessary, and two years after the operation there had been no recurrence.The reasons for the development of this tumour and its histogenetic origin have still not been completely explained. However new immunocystochemical examinations tend to support the neural theory of its occurrence. Thus, this change should be kept in mind during differential diagnostics of changes which resemble mucocele, fibromas, pyogenous granulomas, papillomas, angiomas and planocellular carcinomas. |

| Abstract: |
Toxic reaction is the most common side-effect accompanying the local anesthetic administration. Toxic reaction symptoms may manifest as CNS symptoms or cardiovascular systems symptoms. Initially, the toxic reaction symptoms in CNS undergo the stage of stimulation, followed by the stage of depression. A case of toxic reaction to the usual dose of Cystocain DS for mandibular anesthesia in an 11-year-old girl is presented. The symptoms began with poor general condition, nausea, severe vertigo, pallor and excessive perspiration, followed by clonus-type muscular convulsions, with consciousness preserved. Upon hospitalization, convulsions were interrupted by i.v. administration of diazepam. Blood pressure returned to normal, circulation recovered and normal frequency and depth of breathing were resumed. To date, only one similar case of reaction to Cystocain DS was reported to the WHO. Mere possibility as well as seriousness and risk of the occurrence of such a situation require close therapist's observations of each patient receiving any type of local anesthetics. |
