No similar study on the incidence of orofacial clefts has been conducted in Croatia to date. This study included children born between January 1, 1988 and December 31, 1998, to mothers resident in Croatia. During the 11-year study period there were 525,298 live-births. A review of medical records on live-births from neonatology units and medical documentation from institutions providing surgical treatment for children with orofacial clefts revealed 903 children born with clefts, 24 (2.65%) of them twins, yielding a cleft incidence of 1.71 per 1000 or 1/581 for the study period. When "syndrome" clefts were excluded the incidence of "non syndrome" clefts was 1.56 per 1000 or 1/641. During the study period the incidence of orofacial clefts varied from 1.43 per 1000 to 2.02 per 1000. Considering cleft lip with or without cleft palate (CL±P) and isolated cleft palate (CP) separately, the incidence of CL±P and CP was 1.05 per 1000 and 0.66 per 1000, respectively. According to districts of Croatia, the highest incidence of orofacial clefts was recorded in central Croatia (1.88/1000) and lowest in Lika and north Primorje (0.91/1000). According to sex, there were 56.4% male and 43.6% female children born with orofacial cleft, yielding a 1.29:1 male to female ratio. CL±P was more common in the male population and CP in the female population. |

| Abstract: |
Orthodontic therapy is not always able to completely meet all osmetic and functional requirements. In such cases, better results are achieved by an interdisciplinary approach, to the satisfaction of both the patient and the therapist.Presentation is made of three patients differing according to their dentofacial anomalies, age and sex, in whom professionally stated socmetic and functional requirements could not have been fully met by orthodontic therapy alone, without the aid of cosmetic dentistry. Although prosthetic treatment (e.g., cosmetic ceramic crown) couldhave been chosen in all three cases, it was decided to perform dental crown reconstruction with composite material (coronoplasty), which produced satisfactory results.Case 1: In a 19-year-old girl, discoloration and size variation of the upper central incisor, caused by endodontic therapy and inappropriate composite filling, remained upon completion of the orthodontic treatment for prognathism, presenting as a cosmetic defect. Satisfactory results were obtained by dental crown reconstruction with composite material.Case 2: An 18-year-old girl with a diagnosis of upper lateral incisor hypodontia on the right and rudimentary form of its counterpart underwent orthodontic treatment with rudimentary tooth extraction on the left an space closure by canine mesialization to the site of the upper leteral incisor bilaterally. As corrective canine regrinding to an incisor failed to produce satisfactory results, coronoplasty was performed.Case 3: A 20-year-old man with a diagnosis of overbite and disproportial crown width of mandibular and maxillary front teeth undeerwent orthodontic treatment, which resulted in residual diastema between the upper front teeth due to the smaller size of the upper relative to lower incisors. Dental crown reconstruction withcomposite material was performed in all four upper incisors.The cosmetic component has become a major factor in the psyhology of good appearance,especially in the young, and one of the main tasks of orthodontics and cosmetic reconstructive dentistry. The filling materials currently used on dental crown reconstruction, in cases such as those described , have allowed for excellent resultsto be achieved in both cosmetic and functional terms. |

| Abstract: |
The Introduction defines hypodontia, oligodontia and anodontia, citing their frequency and giving a description of accompanying occurrences. The need for an interdisciplinary approach to the solution of these anomalies is stressed. This is followed by a case presentation of a patient with oligodontia, in whom, apart from the wisdom teeth, 8 permanent teeth were missing. A method of orthodontic treatment is described and final prosthetic rehabilitation. The patient was treated with mobile and fixed orthodontic appliances in order to achieve optimal placement of the teeth and intermaxillary relations, as a prerequisite for successful prosthetic rehabilitation. The prosthetic construction was performed with ceramic bridges in both jaws, which provided a satisfactory functional and aesthetic result. In conclusion the theoretical possibilities for orthodontic treatment and alternative prosthetic solutions in diagnoses in which there is a reduction in the number of teeth, are discussed. |
