| Abstract: |
The aim of prosthetic therapy is to create an illusion of natural health, to establish the physiology and esthetics of the masticatory system and psychological calamity and security of the patient. These goals should always be in focus. Each dentist that uses implant treatment must pose a question whether implant placement is the best treatment choice, or could the situation be resolved with conventional means. Dental implants and implant-borne prosthetics are today accepted as long-term predictable prosthetic-restaurative solution with advantages and disadvantages on scientific and professional levels. Implant-borne prosthetics represent a challenge since a clinician has a possibility to decide on the size and shape of the occlusal surface (scheme); to decide on the number, position, size and orientation of the implants; to modify the quantity and architecture of the bone. Two main etiological reasons that lead to implant failure are bacterial infection and local biomechanical factors that are correlated to implant overload. The role of the occlusion is, therefore, of great importance for different types of prosthetic constructions, especially since it is often disregarded in the clinical work. Natural teeth have periodontal proprioceptors that protect the teeth and their periodontia from the overload that causes trauma of the supporting bone. Although there are many factors included in neuromuscular activities in the natural dentition, there are no specific defense mechanisms to occlusal forces in implant-supported occlusion. Therefore, a poor occlusal scheme on dental implants can lead to failures, but when known prosthetic postulates are used, implants serve with high success rates. The aim of this presentation was to review the occlusal concepts (bilateral, unilateral, canine guidance and “implant occlusion”) that can be used in different prosthetic reconstructions on implants (from single tooth replacement to complete oral rehabilitation). The accent was on biomechanical postulates of the occlusion that are complementing the systematic, individualized treatment plan and precise surgical procedures. |

| Abstract: |
Often wrong judgment of unspecifi c otological symptoms, caused by some types of temporomandibular disorders, is based on the connection in the embryonic development of the temporomandibular joint (TMJ) and the ear. This is a report of a female patient who was previously treated by the ear-nose-throat specialist because of otalgia, but it was concluded that she had some TMJ pathology. Clinical examination and magnet resonance of the TMJs confi rmed anterior slide of the disc. Since the patient has fi ve-year-old complete dentures, new ones were fabricated. The symptoms recede, and for the patient the most important thing was that the pain disappeared. With regards to her age, acquired anodontia and impossible restitutio ad integrum treatment, she was instructed how to support the painless functional status of her masticatory system. |

Aim of this study was to investigate the frequency of symptoms, clinical signs and diagnosis of temporomandibular joint (TMJ) arthrosis in relation to loss of lateral teeth in the population of young males. Hundred and ninety recruits of the Croatian Army, aged 19 to 27 years were clinically examined. Crepitation and pain in TMJ was examined by manual functional analysis. The mandibular excursion was measured by caliper, and loss of lateral teeth by Eichner’s classification. The reliability of clinical diagnostic was crosschecked by means of magnetic resonance. Crepitation in TMJ was observed in 8.9% of the subjects. Osteoarthrosis was determined in 3.6% and osteoarthritis in 3.2% of the subjects. Spearman’s coefficient of correlation rank was significant for following variables: crepitation and pain p < 0,01; clinical signs and: - pain p < 0,01, - crepitation p < 0,01, - reduced movement of the mandible p < 0,01, - osteoarthrosis p < 0,05, - osteoarthritis p < 0,05 and reduced movement of mandible and pain p < 0,01. Eichner’s classification does not show significance in relation to clinical signs of TMJ arthrosis. Frequency of clinical signs of arthrosis in recruits is low as well as the need for treatment. |

Studies have indicated that patients with TMD demonstrate increased somatization, stress, anxiety, depression.A consistent relationship has been demonstrated among anxiety, general somatic complaints, and TMD-related pain. The aims of this study were to determine the differences in depression and somatization scores in patients in different RDC/TMD axis I diagnostic groups and to investigate the role of psychological factors (depression and somatization) in TMD. One hundred fifty- four patients (37 male and 117 female; mean age, 39.0 ± 14.5 years) with RDC/ TMD-defined clinical TMD were selected. Patients were subsequently classified into 7 groups based on the presence of the various RDC/TMD axis I diagnostic groups. Differences in mean SCL-90 depression and somatization scores between the diagnostic groups were compared by one-way analysis of variance and Scheffé post hoc tests at a significance level of 0.05. The frequencies of the different diagnostic groups were as follows: group 1 (muscle disorders, MD), 35.7%; group 2 (disc displacement, DD), 18.2%; group 3 (arthralgia, arthritis, arthrosis, AAA), 7.8%; group 4 (MD+DD), 9.1%; group 5 (MD+AAA), 13.0%; group 6 (DD+AAA), 9.1%; group 7 (MD+DD+AAA), 7.1%. The majority of patients had one diagnosis (61.7%) while the remaining patients experienced two or more diagnoses (38.3%). About 19.5% of TMD clinical patients yielded severe depression scores, and 27.3% experienced severe levels of non-specific physical symptoms scores. Only 6 patients (21.4%) had high disability with moderate and severe limitations (psychosocially dysfunctional patients). Patients diagnosed with myofascial pain and arthralgia (group 5 and 7) had significantly higher levels of depression and somatization than patients diagnosed with only disc displacements (group 2). These data mandate that screening and treatment for depression and somatization should be an integral part of the evaluation and management of patients with TMD. |

Complete loss of teeth means interruption of the physiological, mental and aesthetic harmony of the stomatognathic system. Attempts are made by prosthetic therapy to compensate for the functional inability by complete prostheses. Apart from the renewal of the ability to masticate, natural appearance and normal speech, the intention is to retain for as long as possible the tissue of the stomatognathic system. At the same time to create biological and mental preconditions for maximal acceptance of full prostheses. A full prosthesis is a restoration and does not enable optimal function of all components. Whether the patient will accept the prosthesis depends on the possibilities and desire to adapt to the new situation. Success of therapy with full prostheses depends on the shape, appearance and resorption of the alveolar ridge, muscular action and their influence on the mucus membrane, which is in direct connection with the full prosthesis. In therapy with full prostheses, a lower full prosthesis represents a particular problem, which is a challenge for every prosthodontist. Fabrication of a lower full prosthesis greatly depends on morphological characteristics, which arise in the lower jaw with the loss of teeth. The basis of successful fabrication of a lower full prosthesis, apart from correct use of clinical-technological methods, is knowledge of the anatomy of the lower jaw and surrounding structures, their relationship to the base of the lower full prosthesis and the prosthesis itself. In conclusion, each form of therapy has its advantages and disadvantages. The advantages of the implant borne restoration, are clearly increased retention and stability, contact with the mucous membrane is not primary, and the anticipated stimulation of the bone is periimplantary. The disadvantages are the feasibility with regard to the mental state of the patients, position ion the mucous membrane (control of base stability and essential underlaying) and the strength and efficacy of mastication (is comparatively reduced). Clinical advice. Never work with “unknown” patients. Get to know the patient first through diagnostic protocol, assess his/her cooperation, acquaint yourself with indications and general medical, intraoral, time restricted and mentally conditioned contraindications, in order to successfully carry out implantoprosthetic therapy. |

Orofacial injuries are athletic injuries which are specific according to place and way of occurence. Sports dentistry is a dental discipline associated with the prevention and treatment of the consequences of orofacial injuries during sport activities. Orofacial, and particularly dental athletic injuries, differentiate according to way of occurence from other dental injuries and can be easily prevented. By using an adequate mouthguard a number of dental and periodontal injuries can be considerably reduced. There are different intraoral mouthguards: stock, mouth-formed and custom-made mouthguards. Even though with the stock and mouthformed mouthguards a visit to the dentist is avoided, only custom-made mouthguards offer maximum prevention from orofacial injuries. By its construction and technological features it is the most comfortable for athletes during training and competition. Preconditions for wearing mouthguards are good oral prophylaxis and restoration of all teeth (without caries and periodontopathy). The motherguard is commonly fabricated on the upper dental arch. Mouthguards are manufactured from materials that should satisfy numerous physical, mechanical, biological and functional requirement. Fabrication method of a mouthguard by means of vacuum lamination is presented. The role of the mouthguard is to prevent laceration of the tongue, lips and cheeks by the sharp edges of anterior maxillary teeth, to reduce the risk of injuries to the anterior teeth, to reduce the risk of mandibular or maxillary fractures as well as damage to the posterior teeth and temporomandibular joints, after blows to the interior aspect of the mandible. Athletes should be informed of the possibilities of prevention of their oral health as well as of the existence of slight difficulties which are inevitable during wear of a mouthguard. |

| Abstract: |
Loss of mandibular continuity creates great problems in prosthetic rehabilitation of edentulous patients. The problems resulting from resection of the mandible comprise difficulty in mastication on other mandibular movements due to loss of physiological funcion and coordination of masticatory muscles, as well as decrease in the lower denture foundation. This report presents fabrication procedures of complete dentures in a patient with unilateral partial resection of the mandible. Two functional impressions secured correct impression of the denture foundation and maximum extension of the denture borders. The function of prostheses in static and dynamic conditions was achieved by correctly defined interarch relationships, arrangement of anterior teeth in accordance with aesthetics, phonation and function and arrangment of posterior teeth in accordance with the ligual area, intercuspidation, static occlusion and articulation. After handing in the dentures to the patient, both static and dynamic occlusion were corrected by a remount procedure. A check-up six months after denture fabrication showed that the patient had normal swallowing and speech, control of salivation and satisfactory masticatory function. |

The Research Diagnostic Criteria for Temporomandibular Discorders (RDC/TMD) guidelines, originally developed in the United States, were translated and used to classify TMD patients on physical diagnosis (Axis I) and pain-related disability an psychological status (Axis II) in Croatiana TMD patients. The RDC/TMD Axis I and II findings were compared to those of Swedish , American and Asian TMD patients.One hundred and fifty-gour consecutive patients (117 female and 37 male) referred to the Department of Prosthodontics and Department of Oral Surgery in Zagreb, Croatia, were enrolled in the study. The mean age of the Croatian population was 39 ± 14,5 years. Frequency distributions and descriptive statistics were obtained through the use of SPSS version 10 for Windows (SPSS, Chicago, IL), and chi-square (2-sided) statistical analyses were performed (P < 0,05) to evaluate gender differences.Group I (muscle) disorder was found in 64,9% of the patients; Group II (disc displacement) disorder was found in 31,8% and 27,3% of the patients in the right and left joints, respectively; Group III (arthralgia, arthritis, arthrosis) disorder was found in 21,4% and 26% of the patients in teh right and left joints, respectively. Axis II assessment of psychological status showed that 19,5% of patients yielded severe depression scores and 27,3% yielded high nonspecific physical symptom scores (somatization). Psychosocial dysfunction was observed in 21,4% of patients based on graded chronic pain scores.Axis I and II findings of Croatian TMD patients were generally similar to their Swedich, American and Asian cohorts. In all 4 populations, women of child-bearing age represented the majority of patients. Muscle disorders were the most prevalent type of TMD. A substantial portion of TMD patients were depressed and experienced severe somatization. These rezults suggest that the RDC/RMD guidelines are valuable in helping to classify TMD patients and allowing multicenter and cross-cultural comparison of clinical findings. |

A patient (28 yrs. old) during the last year has noticed wear facets on the teeth and also symptoms of temporomandibular disorders (TMD) - pain, limitation of mandibular movements, and also clicking in the right temporomandibular joint (TMJ). The pain was evaluated with analog-visual scale (0 - 10) = 7. Case history revealed the presence of mental trauma four years ago, which had caused the increase of anxiety and occurrence of panic attacks. By means of manual functional analysis myofacial pain and anterior disc displacement without reduction in the right TMJ was determined and confirmed with electronic axiography and magnetic resonance imaging (MRI). As a consequence of excessive teeth wear the interfering contacts in laterotrusive movements of mandibule were established. Anxiety was confirmed with psychological measuring instrument State-Trait Anxiety Inventory. For the initial treatment the fabrication of Michigan splint was indicated. After 2 months improvement in clinical condition was achieved. Stronger pain was only during larger mouth opening. In the right TMJ crepitation was found. In the last few months she took irregularly prescribed anxiolytic drugs. She still feels anxious, tense and insecure. After 9 months the anterior disc dislocation without reduction and with present signs of subchondral degeneration and condylar aplation in the right TMJ was established with controlling MRI. Orthopedic stability of TMJs was established with definitive occlusal treatment (restoration of wear teeth tissues with composite fillings). Because of accompanying osteoarthritis and better stabilisation of TMJs as well as teeth protection from bruxism a stabilisation splint was fabricated. Bruxism is a pathophysiological disorder of unclear aetiology. The most noticeable sign of bruxism is excessive teeth wear which can cause pathological occlusal relationships. The mechanism of aetiopathogenesis of TMD is insufficiently explained and occlusion is considered to be a secondary aetiological factor. Anxiety can be a pathophysiological factor of bruxism and can lead to persistence of chronic temporomandibular pain. |

Trauma of teeth and soft tissues of the orofacial system are today one of the major health problems. According to the aetiology of dental trauma a separate group are sports traumas which are characteristic for many sports. The protective role of the mouthguard in the preventionof laceration of tongue, lips and cheeks caused by sharp incisal edges, in most cases of anterior superior teeth, decreases possibility of mandibular and maxillary fracture as well as injuries to posterior teeth and temporomandibular joints as a result of a blow in the lower mandibular margin. The fabrication procedure of individual sports mouthguard by modelling technique on plaster casts by vaccuum forming (Erkoform®, Erkodent) is presented. Individual mouthguard is most frequently fabricated on superior dental arch, and the preconditions for wearing the mouthguard are good oral prophylaxis and completely cured teeth. Clinical and laboratory procedures of fabrication comprise the following working phases: preliminary impression of maxilla and mandibule in alginate, transfer of upper dental arch in articulator and centric record, laboratory fabrication, finishing and polishing, delivery to patient, grinding and subsequent care. Individual mouthguard offers most in the prophylaxis of orofacial traumas, and with its construction and tecnological characteristics is the most pleasant for athletes during training and competition. |

| Abstract: |
The Michigan splint covers all occlusal surfaces of the dental arch and is used in initial prosthetic treatment during the diagnosis and treatment of temporomandibular disorders. The most frequent indication is myalgia of the masticatory muscles and arthralgia of the temporomandibular joint. The splint eliminates symptoms and realises self-positioning of the mandible into a new position, which is acceptable for the patient. It can be worn for a longer period with the object of protecting the hard dental tissue due to bruxism. Because of stability and retention on the base the upper dental arch is more frequently chosen. The occlusal surface of the splint is fabricated in the form of flat surfaces on which equal contacts of the working cusps of the antagonistic jaws are achieved in the area of freedom in the centric. In excursive movements of the mandible disocclusion of the remaining teeth is realised, guided by the canine. Interrupted wearing of the splint follows during an interval of gradual disuse and reduction in the period of wearing. Success of the treatment is conditioned by careful fabrication of the splint in the articulator, regular check-ups while wearing the splint and occlusal relations. |

Prosthodontic therapy on dental implants is one of the solutions for partially and totally edentulous patients. The long-term success directly depends on static and microbiota around implants. The purpose of this study was to determine the difference in subgingival aerobic microflora in patients with or without implants as well as evaluate their sensitivity to antibiotics. The investigation consisted of 41 patients. Nineteen patients had inserted implants and twenty-two natural teeth as abutments for fixed bridges. Five different genera of bacteria and one fungus were isolated by smear method from the subgingival areas. The periodont and periimplant area showed great similarity in the genera of microorganisms, despite which Branhamella spp. was found only in patients with implants and Streptococcus spp. and Candida albicans around the teeth. |

| Abstract: |
Control and correction of occlusal relations are a constituent part of clinical and laboratory procedures of complete denture fabrication. Denture materials and fabrication procedures cannot ensure dimensionally correct complete dentures, and therefore it is necessary to check the occlusion. A remount procedure is carried out in order to establish correct occlusal contacts of denture teeth bymounting the finished dentures back on the articulator. There are several reasons for remounting: changed volume of the acrylic resin during polymerisation, dimensional changes in the early days of wearing due to water absorption in the acrylic base and placement of denture bases to the denture foundation area. Remounting startswith fabrication of transfer casts, determination and transfer of interarch relations to the articulator. When the dynamic concept of occlusion is chosen, priority is given to incisor or canine teeth guided occlusion. Remounting should be a constituent part of complete denture fabrication. Mandibular overdentures, supported by osseointegrated implants, is a well established, successful treatment of partialy edentulous patients. The aim of the study was to analyse the bone mineral density (BMD) in the implant site of the mandibles with ITI implant-supported overdentures when compared with the same site of the edentulous mandibles in complete denture wearers. Eight patients (4 males, 4 females) with 16 ITI implants and overdentures in the mandible and 8 complete denture wearers (4 males, 4 females) who had approximately the same body mass index and the same age participated. The BMD measurements were performed on digitised periapical radiographs with a 10 steps copper stepwedge attached to each film. Grey levels of each step of the stepwedge were transformed to optical density values and using the 3rd degree polynomial the regression formula was calculated for each film. The BMD values of each measured region of interest (ROI) were expressed in the copper stepwedge thickness equivalents. BMD values of the ROIs close to the implant were statistically comparedto the same ROIs in the edentulous mandible using t test for independent samples. BMD values were significantly higher in ITI implant ROIs compared to the matching edentulous mandible sites (p<0.05). We concluded that an increased function after the implant-overdenture treatmentcaused a load-related bone formation which minimizes the physiologic age-related mandibular BMD loss.Supported by the Ministry of Science and Technology, Republic of Croatia, Project No. 065911. |

Edentulousness is a considerable problem in Croatia. So far prevention has not become the most important part of the dental profession. On the other hand,poor medical knowledge, reduced rights concerning health insurance costs as well as an increasing number of impoverished people in Croatia has resulted in postponed prosthetic rehabilitation. For the above mentioned reasons the Croatian people suffer from premature loss of their teeth. Also lower jaw atrophy occurs, which makes prosthetic rehabilitation even more difficult to achieve. In spite of some disadvantages, the double-implant borne prosthetic suprastructure has proved to be a simpleand good solution to the patient's problem, mainly because it is cost-effective. This particularly applies to Croatia patients. Over the last five years we have placed double -implants in 26 patients, in the anterior region of the mandible. The implants were placed in the region of the lower canine or slightly more mesially. Severe atrophywas determined in 13 patients (50%) which impeded their complete denture wearing even before the implant placement started. However, we made up for the loss in two patients by placing the implants again. This time we placed them slightly more mesially.We made one borne implant complete denture for one patient because the examination revealed severe atrophy in one segment of his mandible. In addition since the osseointegration prognosis for this patient was questionable we decided against any additional surgical treatment. Since the belts of the attached gingiva in our patients were wide enough and the diameters of the implants were not very long, no vestibuloplasty was necessary. We installedITI, IMZ, ASTRA and Ankylos implants. All systems proved to be equally functional. |

PURPOSE. The aim of this study was to register and measure lower jaw movements and to analyse the measured length of maximal right and left mediotrusion movement in asymptomatic and symptomatic subjects.MATERIALS AND METHODS. A symptomatic group consisted of 51 subjects with temporomandibular disorders. A control group consisted of 43 subjects without signs and symptoms of temporomandibular joint disorders. In the symptomatic group of subjects signs and symptoms of temporomandibular disorders were crepitation, bruxism, sensitivity, pain in the temporomandibularjoint and muscles, as well as pain and sensitivity in the region surround and anterior to the ear, together with difficulties while opening the mouth. Each subject was registered by the GAMMA CADIAX system for registration of positions and movement of the lower jaw, which consist of a convetional SAM axiograph, electronic device for drawing of curves with a computer.RESULTS. No significant differences were found between the groups of subject for the measured variables. CONCLUSION. The results of the length of the mandibular and condyle movements are important, alhough unreliable indicators of temporomandibular join function. Description analysis of a graphic recording of mandibular and TMJ movement remains a precise evaluation method for determination of TMJ dysfunction. |

The significance of occlusal interference in the etiology of temporomandibular disorders has been questioned in numerous recent articles. The aim of this study was to determine the prevalence of the clinical signs and symptoms of temporomandibular disorders in a young male nonpatient population and to investigate a possible association between the signs and symptoms of temporomandibular disorders and occlusal interference. A questionnaire including data from history and clinical functionalexamination was used in the study. All subjects (a total of 230) were male (army recruits), of 19 to 28 years of age (mean 21.3). Temporomandibular joint clicking was reported in 91 subjects, temporomandibular joint pain on palpation and functional loading in 78 subjects, masticatory muscle pain on palpation and functional loading in 58 subjects, tension type headache in 30 subjects, andmandibular deviation on opening and closing movements greater than 2 mm in 43 subjects. The prevalence of occlusal interference in percentage in 230 young adults, 65% had no occlusal interference during examination of the functional state of occlusion, while 14%subjects had centric slide between centric relation and maximum intercuspation, 5% subjects had working side interference and16% subjects had non-working side interference during lateral and protrusive mandibular movements. Clinical signs and symptoms were correlated with occlusal interference, although their correlation cannot be considered unique or dominant in definition of a temporomandibular disorder population. |

Patients often wear inadequate complete dentures that become functionally unacceptable due to marked abrasion of the prosthetic teeth and resorption of the denture bone foundation. Symptoms of dysfunction in the temporomandibular joint may also occur. Because of the uncontrollably reduced vertical dimension of complete dentures and the treatment of the signs of temporomandibular dysfunction, as confirmed by magnetic resonance imaging, wearing of a stabilization (vertical) splint is indicated. The splint is made in an articulator where occlusal relationships are checked and corrected. During the therapy the patient’s subjective symptoms and objective findings are monitored. Successful initial therapy is a precondition for success in final prosthetic treatment by complete dentures. |

The corrosive process of dental alloys is very well known, although little information is available on the corrosion of dental ceramics. The aim of this study was the analysis of the chemical stability of two dental ceramics in an aggressive acid medium. Two samples of feldspathic ceramic fused to Co-Cr and Ni-Cr alloy and a sample of hydrothermal ceramic fused to high-noble gold alloy were deposited in 10-3 mol/L HCl at 50°C during six months. The solutions were analysed after one, two, three and six months. The determination of Na+, K+, Ca2+ and Mg2+ was performed by ion chromatography (IC), the quantities of the eluted Si4+ and Al3+ were determined by spectrophotometric method and the weight loss of each sample was measured by an analytical scale. The changes of the pH values were determined by means of a pH meter. The results showed that most ions were eluted after the first three months, while after that period of time elution was significantly lower in each sample. The sample with hydrothermal ceramic showed the most significant elution and the highest weight loss. The pH values changed in each leachate sample, but stabilised after the 3rd month of the investigation. It can be concluded that dental ceramics are not stable in an acid medium. |

| Abstract: |
Correct occlusal relationships on complete dentures are part of the success in prosthetic treatment of edentulous patients. As denture base materials and fabrication procedures cannot provide dimensionally accurate complete dentures, it is necessary to check the occlusion. The relation between artificial teeth on complete dentures has a significant impact on even loading of denture foundations and optimum stabilisation of dentures during wearing. A remount procedure begins with fabrication of remount casts, determination and transfer of interarch relationships into the articulator. Deflective contacts on dentures are eliminated by selective grinding carried out in the articulator in the intercuspal position and by excursive tooth guided movements. The purpose is to represent a remount procedure as a constituent part of complete denture fabrication procedures in everyday practice. When selecting a dynamic concept of occlusion, priority is given to unbalanced dentures, i.e. to canine guided occlusion and/or to the concept of unilateral occlusal balance. |

An immediate complete denture is a restoration for lost natural teeth and associated tissues, which is inserted into the patient's mouth immediately following the extraction of the remaining teeth.The purpose of the paper is to draw the dentist/practitioner's attention to the need for immediate complete denture fabrication. The aims, advantages and disadvantages of immediate dentures as well as contraindications are described in detail.Modern procedures for immediate complete denture fabrication as well as instructions to patients about wearing the denture and mouth and denture hygiene are given.A need for a timely rebasing of immediate dentures and adjustment of the occlusion is also pointed out. The functional, aesthetic and psychological success of immediate dentures depends on correct indication, diagnosis, treatment planning and precisely executed fabrication procedures. |

| Abstract: |
The aim of the study was to determine whether the rate of cooling induced structural changes in a high-gold alloy, and whether these changes modified the bond strength between the gold alloy and hydrothermal ceramic. Before ceramic firing, the casts were submitted to fast (water), normal (air) or slow (in the furnace) cooling. Three-point bending test was used in a total of 15 specimens of defined dimensions. The ultimate force at which ceramic detachment from the metal surface occurred and bending strength were determined. The values of coefficient (k) and surface (S) as factors in the assessment of bond quality were calculated. Photographs of the specimen microstructure were made and values of the microhardness of both materials determined. Analysis of variance (ANOVA) revealed no statistically significant difference in bond strength among the three types of specimens, suggesting that the mode of alloy cooling had no substantial effect either on the microstructure of the high-gold alloy or on the quality of bond between the gold alloy and hydrothermal ceramic. |

The aim of the study was to evaluate patients’ general satisfaction with their removable partial dentures of different classification, construction, material, denture base shape (major connectors), denture support and the number of missing teeth. The aim was also to evaluate patients’ satisfaction with denture retention, speech, aesthetics, chewing ability and the comfort of wearing dentures, depending on the construction. A total of 165 patients with partial dentures took part in this study. There were 59 males and 106 females aged from 38 to 87 years. Patients graded satisfaction with their partial dentures in total and also with aesthetics, speech and retention, chewing ability and the comfort of wearing dentures by means of a scale from 1 to 5. The dentist determined Kennedy classification and their modifications, material and denture support, denture base shape and the number of missing teeth. The dentist also evaluated the denture construction. On the basis of the statistical analysis the following conclusions were made: 1. Patients were on the whole satisfied with their partial dentures (distribution of the scores of the patients’ assessments was asymmetrical towards the highest scores in all examined categories). 2. Kennedy classification and their modifications, material, denture base shape and denture support do not have an influence on patients’ satisfaction with denture retention, speech, mastication, aesthetics and comfort of wearing dentures (p>0.05). 3. The number of lower missing teeth influence the comfort of wearing lower dentures (p<0.05). 4. Construction of lower dentures have an influence on patients’ satisfaction with speech with lower partial denture (p<0.05). |

Owing to the ample dental market supply, more than a thousand different systems and alloys for prosthetic use are presently available. The aim of the study was to identify the basic components of the existent prosthetic constructions in patients requiring additional prosthetic therapy. Anodic sampling of eight prosthetic restorations of unknown composition was performed using a minielectrode with a graphite handpiece, aqueous NaCl solution as electrolyte, and a 4.5 V battery as a source of electric current. The samples were analyzed by quantitative thin-layer chromatography (QTLC). Sample chromatograms were developed by use of the acetone and 2 M HCl solvent system. Cobalt was visualized using aqueous solution of sodium-diethyldithiocarbonate, and chromium by ethanol solution of dimethylglyoxime. The percentage proportion of the elements identified was determined on an image analyzer, previously gauged according to the known standards. |

Contradictions in the field of epidemiology, etiology, diagnosis and treatment of temporomandibular disorders continue, especially because such disorders rarely appear as individual entities, but as multiple problems with overlapping signs and symptoms. The purpose of this study was to establish the prevalence of temporomandibular disorders in a population of younger male subjects (nonpatients) in relation to interarch relationships in the intercuspal position, to establish a correlation between signs and symptoms and morphological occlusion(Angle classification) and to compare results with known clinical studies. The study comprised 230 male subjects aged from 19 to 28 years, who were examined based on a questionnaire (clinical functional analysis according to Gutowski).From the total number of subjects, 44.3% of subjects had at least one sign, and 38% of subjects had at least one symptom of a temporomandibular disorder. Analysing signs and symptoms of temporomandibular disorders individually, 91 subjects (39.6%) had jaw joint clicking, 3 subjects (1.3%) had crepitation in jaw joints, a total of 78 subjects (33.9%) had pain in jaw joints at palpation and functional loading, a total of 58 subjects (25.2%) had pain in the muscles of mastication at palpation and functional examination, 43 subjects (18.7%) had a lower jaw deviation exceeding 2 mm and 30 subjects (13%) had tension-type headache. By the calculation of Spearman’s correlation rank, a correlation between signs and symptoms of temporomandibular disorders and interarch relationships in the intercuspal position, especially in the group with disturbed interarch relationships (class II/1, class II/2, class III and crossbite), wasestablished. |

Disinfection of elastomeric impression materials used in prosthetic dentistry can cause dimensional changes in the impression materials. 0.5% chlorhexidine gluconate is usually used for disinfection of impressions at the School of Dental Medicine in Zagreb. The purpose of this study was to measure and compare linear dimensional changes of three elastomeric impression materials after their immersion in a certain disinfectant.The master dies were made according to the ADA specification no 19. The tested materials were Panasil (addition silicone), Blend-a-scon (condensation silicone) and Impregum F (polyether). Twelve samples were immersed in disinfectant, twelve in water and twelve left in the air for four different periods of time (10 min., 30 min., 60 min and 24 hours). Linear dimensional changes were measured by a non-contact digital tool microscope.The smallest changes were noticed in addition silicone with the least tendency to expansion. The condensation silicone contracted within a tolerant range of 0.2 - 0.4%, according to DIN 13913. Polyether expanded in the disinfectant significantly.Our results show that the tested polyether is not appropriate for disinfection by immersion in chlorhexidine gluconate, while the other two materials underwent 24-hour immersion with linear dimensional changes within a clinically acceptable range. |

| Abstract: |
By preserving the anatomic integrity of the stomatognathic system, a prosthesis allows its physiologic activities to proceed normally. A function imprint provides a picture of the prosthesis support and relationship between mobile tissues and prosthesis rims. A high-quality function imprint required, but current literature offers different opinions and views concerning the choice and method of taking a function imprint. Several procedures covering all methodologic differences can still be singled out: taking a function imprint without pressure, taking a function imprint with pressure, taking a function imprint with the mouth open, taking a function imprint with the mouth closed. The regions with a thin and less keratinized mucosa over the bone but without a submucosal interlayer must be relieved or imprinted without pressure. Forward inclination of the head and the position of the tongue make the soft palate to acquire a functional position on taking the imprint, thus allowing the posterior pharyngeal rim of the upper prosthesis to be determined. The greater the valve effect and the lesser the space between the prosthesis base and support, the higher the negative pressure upon the support. Imbibition is achieved by the prosthesis rims lying in the initial part of mobile mucosa, and the pharyngeal rim on the border between the hard and soft palate, in the region of the "ah" line. Retention of the prosthesis is mostly secured by its posterior, dorsal patal or pharyngeal rim. When pointing to its importance in retention, it applies to the rim margins, its relation to the support and its role in the valve closure of the upper total prosthesis. |

| Abstract: |
In this study, teleroentgenocephalograms of 34 subjects with at least 20 natural teeth were used. Correlation between the angle formed by linking the PORION-NASION-SPINA NASALIS ANTERIOR (PONANS angle) points and the angle made by Frankfort horizontal and occlusion plane was studied. The coefficient of correlation was 0.1263. It is considered that there is no correlation if the coefficient of correlation is less than 0.5. Thus, there was no correlation in the study sample, which is consistent with the results reported by Karkazis and Polyzois, but in disagreement with those published by Monteith. In our sample, the occlusion plane could not be oriented in the articulatory space or in toothless mouth by means of the method proposed by Monteith. |

| Abstract: |
A skin projection of the orbitale as palpated through soft tissues was designated on the left cheek of 22 subjects. The nasion, orbitale and cephalostat nasion direction medium were plotted on transparent paper after teleroentgenography. Mutually parallel lines were drawn through the points, the line obtained on the construction of the nasion direction medium used as a basic one. Distances between the lines were measured and results statistically processed. Transfer of the model into the articulatory region by means of the orbitale and buccal arch nasion direction appeared not to orientate reliably the model according to reference planes of the head. Our results are consistent with literature data available. |

| Abstract: |
Positions of the central relation and central occlusion were intraorally graphically recorded in 100 patients with a stomatognathic system dysfunction and in 0 control subjects. Results of the study showed asymmetry of these two positions to play a major role in the etiology of dysfunction. Occlusal program had an almost two-fold impact on the graphic tracing of the central occlusion position, as compared to the influence exerted by the articulation and muscle on the central relation tracing. Instrumental functional analysis and the instrument used alike, are simple to use and allow an objective assessment of the state to be made. |
