HISTORY OF DENTISTRY DENTISTS PORTRAITS IVANIS, TOMISLAV |
| Abstract: |
On 11 April 2001 Prof. Tomislav Ivaniš, Professor at the School of Dental Medicine, and Head of the Department for Fixed Prosthetics of the Clinic of Dental Medicine, Clinical Hospital Centre Zagreb gave up his fight against illness. Lost was the last trace of hope for our colleague and friend, who incredibly bravely bore his suffering for so long, conscious and ready to share a joke until the very end.Professor Ivaniš was born in Zagreb on 21 September 1937 where he finished primary and high school, and also primary and middle music school. Apart from his great love of the piano he also had a marked inclination for art. He was also known to all of us as a great supporter of the “Dinamo” Football Club. His professional career started in 1963, after graduating at the School of Dental Medicine in Zagreb, as Assistant at the then Department of Dental Prosthetics at the same School. |

| Abstract: |
A survey, which consisted of 21 questions, was conducted during 1999/2000 on 135 dentists in Croatia. Our aim was to objectify dentists’ knowledge of HIV infection. A questionnaire was divided into four groups, as following: general knowledge of HIV, protection and infection control, risks while treating HIV infected patients, and willingness to treat HIV infected patients. Only 40% of surveyed dentists know basic facts of HIV, oral manifestations and modes of transmission, and 50% of all surveyed dentists use protective masks, gloves and eyewear. More than 49% overestimate risks while treating HIV infected patients. More than 55% of all surveyed dentists are willing to treat HIV infected patients. Results of this survey show informational gaps in dentists’ knowledge of HIV infection and highlights the need for additional lectures on HIV infection. |

| Abstract: |
Knowledge of the dynamics of growth and eruption of teeth during the period from mixed to permanent dentition is essential as the majority of malocclusion begins at this time, and requires prompt orthodontic treatment. The aim of this study was to determine the level of the position of tooth germs and their zones of resistance in relation to the occlusal plane, in order to determine potential differences between male and female subjects and to conclude whether differences exist in the level of the germs and order of eruption in the examined groups of teeth. The sample comprised 200 orthopantomograms (96 boys and 104 girls aged from 6 to 10 years) at the Department of Orthodontics School of Dental Medicine University of Zagreb. The shortest distance of the canine and both premolars to the occlusal plane was measured directly on the X-ray films. Based on the results of the study it can be concluded that in boys, in both jaws during the defined development phase, the premolar was nearest to the occlusal plane, followed by the second premolar and then the canine. In girls, in the upper jaw, the first premolar was the nearest to the occlusal plane, followed by the second premolar and then the canine, and in the lower jaw the premolar was first, followed by the canine and the second premolar. In both sexes the position of the corresponding teeth was symmetric in the same jaw, and an identical order of the teeth eruption can be expected. Examination of the differences in arithmetic means for all variables according to sex confirmed statistical significant differences for the canine, and other differences were incidental. |

DENTAL SCALING ROOT PLANING PERIODONTITIS + microbiology |
| Abstract: |
The occurence of periodontal pathogens in subgingival flora in periodontitis is a risk for periodontal disease progression. Therefore microbiologic diagnostic procedures are justifiably indicated in the detection of pathogens, monitoring of therapy success and outcome of the disease. The aim of this study was to show the effect of scaling and root planing on clinical and microbiological factors in 28 patients with chronic and aggressive periodontitis. Clinical assessment and microbiological testing were performed prior to, and three months after mechanical therapy. The presence or absence of bacterial plaque, gingival bleeding, pocket depth and attachment loss were assessed before and three months after scaling and root planing. Samples of subgingival plaque taken from periodontal pockets, were analysed by polymerase chain reaction technique for the presence of seven bacterial pathogens. Results of clinical parameters and bacterial prevalence were analysed before and after therapy by Wilcoxon Rank test. The mean pocket depth significantly decreased from 3.9 to 3.0 mm. Clinical attachment level decreased moderately from 4.1 to 3.8 mm. Mean plaque and gingival bleeding values also decreased after therapy. The prevalence of subgingival pathogens in relation to subjects was as follows: only one pathogenic species was found in 28.6%, two were found in 46.4% and three in 14.3% of subjects. The most prevalent pathogens were bacteroides forsythus in 85.7%, Porphyromonas gingivalis in 32.1%, Actinobacillus actinomycetemcomitans and Fusobacterium in 32.1% of subjects. After therapy the prevalence of pathogens decreased moderately. The total number of tested pathogens decreased in 12 subjects and this result was statistically significant. (p=0.001). In 16 subjects the number of pathogens was the same, and did not increase in any of the subjects. The results indicate that the effect of scaling and root planing in the treatment of periodontitis was effective in achieving clinical and microbiological improvement by decreasing the prevalence of pathogens responsible for disease progression. |

INLAYS COMPUTER-AIDED DESIGN |
| Abstract: |
The CAD/CIM method is a new technology in dental medicine which was first introduced in 1996. The paper presents the development of technology and its possibilities. The method of application and fabrication of inlays and veneers is presented and advantages and disadvantages compared to existing conventional methods. The main advantages of the method are that fabrication of an inlay and veneer can be carried out during one appointment in the dental surgery, technicians work in laboratory is unnecessary and the fabricated inlays and veneers are of standardised quality. Phases of the fabrication of an inlay and its final appearance in the mouth are presented. In conclusion it is emphasised that the method offers the possibility of rapid and relatively simple fabrication of inlays, comparable in quality to those produced in the laboratory. Further investigations are needed in order to solve the problem of the width of marginal fractures and application of optimal materials for cementation of the inlay, and to achieve the best possible clinical function of inlays and veneers fabricated by CAD/CIM technology. Clinical results indicate that this technological method has a bright future for use in contemporary dental practice. |

BONE RESORPTION |
| Abstract: |
The problem of healing bone defects of odontogenetic aetiology is the most frequent cause of failure in surgical treatment of an ostitic process. The aim of this study was to valorise the successfulness of healing of bone defects after implantation of a new alloplastic copolymer - polyglycol bone implant (Fisiograft). A group of 45 subjects was examined with an ostitic process on teeth of the intracanine region. The results were valorised on the basis of densitometric measurement over a period of 12 months after implantation. The results obtained indicate that polyglycol copolymer bone implants can be successfully used in the treatment of bone defects of odontogenetic aetiology. Their fundamental advantage is slower biodegradation, which ensures a more suitable area for the apposition of new bone in the lumen of the bone defect, simple application in clinical work and the possibility of a mutual combination of all three available forms. |

| Abstract: |
The aim of the study was to evaluate patients’ general satisfaction with their removable partial dentures of different classification, construction, material, denture base shape (major connectors), denture support and the number of missing teeth. The aim was also to evaluate patients’ satisfaction with denture retention, speech, aesthetics, chewing ability and the comfort of wearing dentures, depending on the construction. A total of 165 patients with partial dentures took part in this study. There were 59 males and 106 females aged from 38 to 87 years. Patients graded satisfaction with their partial dentures in total and also with aesthetics, speech and retention, chewing ability and the comfort of wearing dentures by means of a scale from 1 to 5. The dentist determined Kennedy classification and their modifications, material and denture support, denture base shape and the number of missing teeth. The dentist also evaluated the denture construction. On the basis of the statistical analysis the following conclusions were made: 1. Patients were on the whole satisfied with their partial dentures (distribution of the scores of the patients’ assessments was asymmetrical towards the highest scores in all examined categories). 2. Kennedy classification and their modifications, material, denture base shape and denture support do not have an influence on patients’ satisfaction with denture retention, speech, mastication, aesthetics and comfort of wearing dentures (p>0.05). 3. The number of lower missing teeth influence the comfort of wearing lower dentures (p<0.05). 4. Construction of lower dentures have an influence on patients’ satisfaction with speech with lower partial denture (p<0.05). |

PERIODONTAL DISEASES + classification |
| Abstract: |
The problem of classification of periodontal diseases is extremely difficult and complex. When the development of periodontology is analysed it can be seen that different attempts have been made to change not only terminology in periodontology but also classification of periodontal diseases. This frequently resulted in a large number of individual systems of classification, while those that were generally acknowledged appeared during the last ten years. Consequently the aim of this study was to present the latest generally accepted classification, proposed by acknowledged world experts during the International Workshop for Classification of Periodontal Diseases and Conditions, in 1999. |

| Abstract: |
The pregnant woman and the nursing mother present a special set of concern for the dental practitioner. Many drugs when taken by a pregnant patient or a breastfeeding mother can pass the placenta or breast alveolar cells and enter the bloodstream of the fetus or the human milk, adversely affecting fetal development or newborn child. The effect that a medication has on a fetus or a newborn child depends on the type of drug, the amount of drug taken, the duration of therapy and in pregnancy the trimester during which the drug was taken. This is a brief review of the relative risks of drugs commonly used in dental care for the pregnant patient and the nursing mother. A list is given of possible harmful drugs in pregnancy and drugs contraindicated during breastfeeding. |

| Abstract: |
The Introduction defines hypodontia, oligodontia and anodontia, citing their frequency and giving a description of accompanying occurrences. The need for an interdisciplinary approach to the solution of these anomalies is stressed. This is followed by a case presentation of a patient with oligodontia, in whom, apart from the wisdom teeth, 8 permanent teeth were missing. A method of orthodontic treatment is described and final prosthetic rehabilitation. The patient was treated with mobile and fixed orthodontic appliances in order to achieve optimal placement of the teeth and intermaxillary relations, as a prerequisite for successful prosthetic rehabilitation. The prosthetic construction was performed with ceramic bridges in both jaws, which provided a satisfactory functional and aesthetic result. In conclusion the theoretical possibilities for orthodontic treatment and alternative prosthetic solutions in diagnoses in which there is a reduction in the number of teeth, are discussed. |

| Abstract: |
Chronic periapical lesion is a result of the mutual activity of microbiota in the root canal and multilateral response of the host to infection. Nonspecific, acute and chronic inflammations, including humoral and cellular immunological responses, participate in the occurrence, development and perpetuation of these lesions. Biopsy samples of periapical tissue were taken by surgical procedure in 34 patients, with the object of verifying immunoglobulin classes G, A and M. The study involved determination of total proteins by Lowry’s method. Concentrations of IgG, IgA and IgM were determined by the method of radial immunodiffusion. Their correct amount was obtained by placing them in relation to the verified amount of total proteins. The study revealed different percentages of the share of immunoglobulins, classes G, A and M, and diversity in their values. IgG was found in 100% of cases, IgA in 76% and IgM in 44%. Mean value of the quantity of IgG amounted to 385.2 mg/g protein, ranging in value from 27.3 to 826.0 mg/g protein, IgA 51.0 mg/g protein, ranging from 0 to 336.8 mg/g protein, and IgM 20.3 mg/g, ranging from 0 to156.2 mg/g protein. The results obtained on the basis of this study indicate the local synthesis of immunoglobulins in chronic periapical lesions, and the involvement of nonspecific inflammatory and specific immunological reactions, which together are responsible for the pathogenesis of these lesions. |

| Abstract: |
Although over 300 bacterial species make up the oral flora, it is thought that only a few, either alone or in combination, initiate the progression of periodontitis. For over 20 years, culture techniques have been the primary method of identifying and studying putative pathogens. Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis, Bacteroides forsythus and Treponema denticola are considered bacterial pathogens. Culture techniques enable versatility in characterizing the subgingival flora, allow for speciation and antibiotic susceptibility testing. Selective culturing involves the use of media restrictive to certain microorganisms, while nonselective media provides maximal growth and captures a predominant cultivable flora. In addition to technical problems, cultivating microorganisms can be both time consuming and costly. Molecular diagnostic techniques, DNA probes and polymerase chain reaction are especially useful in detecting those bacteria and viruses that cannot be cultivated in vitro or are not sensitive to current cultivating techniques. Sensitivity and specificity is optimal according to the great number of bacteria present in plaque samples. |
