| Abstract: |
The priority of each dentist should be the health and care, as well as the welfare of his patient. The dentist should explain the therapeutical process, inform the patient in detail about the chosen therapy and keep him updated. The patients, usually lacking professional knowledge, or insufficiently informed, are unable to choose the type and the course of the therapy. The role of the dentist is to prepare the therapy plan, as well as the rehabilitation of the patient, and to instruct him as to the desired results.Preparing the therapy plan is a very demanding part of the process, which necessarily unites practical knowledge and scientific and socio -psychological approach to the patient, medical ethics included. |

| Abstract: |
Dental ceramics justifies more frequent use in prosthetic restoration of damaged dental status. Inlays, crowns and three-unit bridges have been made of all-ceramic system. Zirconia dioxide is a well- known polymorph. The addition of stabilising oxides like MgO, Y2O3 to pure zirconia, makes it completely or partially stabilized zirconia which enables use in prosthodontics. Tetragonal Zirconia Polycrystals (TZP) stabilized with 3mol % yttria, has excellent mechanical and esthetical properties. Fixed prosthetic appliances of this ceramic have been made using CAD/CAM techniques. It can be expected that zirconium oxide ceramics will replace metal-ceramics in restorations that require high strength. |

Inflammatory periodontal diseases present a global health care problem and are one of the main reasons for tooth loss. We investigated success of the initial periodontal therapy in patients with chronic (CP) and aggressive (AgP) periodontitis, and compared clinical outcome between these groups. We used periodontal pockets probing depth before and after the therapy, being the most important clinical index for prediction of further disease progression. Investigation included 35 patients with AgP, and 71 patients with CP. In the AgP group the therapy included systemic antibiotics and local irrigation with chlorhexidine solution. Subjects with AgP had significantly deeper pockets than subjects with CP (4.49 ± 0.93 mm AgP group, 3.87 ± 0.78 mm CP group). The initial therapy significantly reduced periodontal pocket depth in both groups (p < 0.001). AgP subjects showed significantly higher probing depth reduction than CP subjects (1.05 ± 0.80 za AgP i 0.55 ± 0.69 za CP). When deep and shallow pockets were separated, the difference between the groups was not significant. The combination of mechanical and antimicrobial therapy in the AgP subjects improved clinical periodontal findings, provided good clinical results, and can therefore be used as a clinical protocol for the initial periodontal therapy of these patients. |

Corrosion of prosthetic restorations and simultaneous wear during usage leads to damage of the restoration and harmful reactions on the surrounding tissues or even on distant organs. The aim of this study was to identify the ions Au, Pt, Ag and Pd in the gingival fluid of patients with fixed prosthetic appliances and to determine the mass of ions in the obtained sample . The study included 20 subjects: of which 15 had fixed prosthetic appliances and the same level of hygiene, and 5 without have any prosthetic appliances, who served as a control group. The prosthetic appliances were made from alloy compounds: Au - Pt, Au - Pt, Ag - Pd, Ni - Cr, fixed in the mouths: from 1-14 days, and 3-8 years. Samples were obtained by placing adsorbent paper (AP) in the gingival sulcus (GS) of the abutment tooth for a period of ten minutes. AP with samples gingival fluid (ST) were analysed by means of a mass spectrometer with fused plasma (ICP-MS). The results were statistically analysed by the programme packet SPSS, with descriptive statistics and correlation of samples. In the whole sample statistically significant correlation was found for the concentrations of Ag, Au and Pt, while the concentration of Pd was characteristic for each sample. Analysis of the results in the control group did not reveal statistically significant correlation for the same inter-relations. In the sub-group Ag-Pd alloys statistically significant correlation was established between the concentrations of Ag, Au and Pd, and the most marked correlation found was between the concentrations of Au and Pd. Statistically significant differences were determined in the concentrations of Pt between the subgroups with new appliances, with the highest value in the group of Au alloy (P=0.023) in comparison with the control group. Statistically significant changes were determined in the values of Ag (P=0.011) and Pt (P=0.039), with a fall in the values of both metals in subjects with older prosthetic appliances. |

No similar study on the incidence of orofacial clefts has been conducted in Croatia to date. This study included children born between January 1, 1988 and December 31, 1998, to mothers resident in Croatia. During the 11-year study period there were 525,298 live-births. A review of medical records on live-births from neonatology units and medical documentation from institutions providing surgical treatment for children with orofacial clefts revealed 903 children born with clefts, 24 (2.65%) of them twins, yielding a cleft incidence of 1.71 per 1000 or 1/581 for the study period. When "syndrome" clefts were excluded the incidence of "non syndrome" clefts was 1.56 per 1000 or 1/641. During the study period the incidence of orofacial clefts varied from 1.43 per 1000 to 2.02 per 1000. Considering cleft lip with or without cleft palate (CL±P) and isolated cleft palate (CP) separately, the incidence of CL±P and CP was 1.05 per 1000 and 0.66 per 1000, respectively. According to districts of Croatia, the highest incidence of orofacial clefts was recorded in central Croatia (1.88/1000) and lowest in Lika and north Primorje (0.91/1000). According to sex, there were 56.4% male and 43.6% female children born with orofacial cleft, yielding a 1.29:1 male to female ratio. CL±P was more common in the male population and CP in the female population. |

| Abstract: |
Surgery of head and neck tumours is complex and unequivocally expensive. There have been very few investigations to date in the world on the cost of treatment of this group of patients, and in Croatia not one. The aim of the present study was to determine the cost of treating patients with head and neck tumours, its structure and factors which have a significant influence on it. The study included 248 patients, treated in the Clinic of Maxillofacial Surgery in Zagreb during the period 1995 - 1996. The cost of treatment is presented as the total cost of hospital expenses, diagnostics, operations, anaesthesia and radiotherapy, and its dependence on all factors tested which we assumed to have a significant effect on it.The average cost of treatment for the patient with head and neck tumours is 17,297 kn. The duration of hospitalisation, type of ablation and reconstructive surgical procedure has the greatest influence on the cost of treatment. Thus, intervention in this segment would enable the greatest savings. As the duration of treatment and type of therapy is decided by the physician, the planning and realisation of cheaper treatment is the responsibility of the physician. |

Leakage examination was performed using fluid transport system measuring movement of an air bubble. Tested techniques were cold lateral condensation technique, Touch`n Heat and Thermafil technique. Root canals were obturated with gutta-percha in combination with AH Plus and RoekoSeal sealers. After storing in saline solution, microleakage was measured in intervals of one month, six months and one year. The minimum microleakage values were obtained in all intervals in canals obturated using cold lateral condensation technique, while maximum values were obtained in Thermafil technique (p< 0.05). Significantly more leakage was found for all tested techniques in the interval of 12 months after obturation (p< 0.05). There was no difference between obturation quality between RoekoSeal and AH Plus, no matter what obturation technique was used. |

The aim of the investigation was to evaluate the quality of prosthodontic appliances relative to chronic organ system dysfunctions and the level of independency in a group of elderly individuals. The study group included 175 elderly home residents in the Zagreb region over 60 years of age (average 76.8 years, S.D. 10.72), which consisted of 66 male (37%) and 109 female (63%) individuals. The examination included dental status, oral and general health history. Quality of prosthodontic appliances was evaluated according to the CDA index for fixed, and the modified Nevalainen et al. index for removable prosthodontic appliances. Assessment of functional independency of an individual was done using the Activity of Daily Living index by Miura, Araki and Umenai. Over 82% of individuals were in need of fixed, removable or combined prosthodontic treatment. The most common general health conditions involved cardiovascular (29%) and locomotor (18%) systems. Correlation was determined between the poor state of prosthodontic appliances and chronic conditions of CVS (r=0.54, p<0.05) and locomotor (r=0.69, p<0.05) systems, psychological (r=0.88, p<0.05), neurological (r=0.72, p<0.05) and digestive (r=0.65, p<0.05) dysfunctions. Spearman's correlation analysis determined significant correlation between the ADL and the CDA indices scores (r=-0.468, p<0.01), and the ADL and the modified Nevalainen's et al. indices scores (r=-0.572, p<0.05), where lower ADL scores were correlated to the poorer condition of the prosthodontic appliances. |

| Abstract: |
This article reviews different types of membranes for guided tissue regeneration. They are used to cover defects and stimulate regeneration of osseous defects in periodontal pockets. A membrane should be biocompatible, enable cell exclusion separating the gingival flap from the fibrine clot and guard space for the new alveolar bone and the periodontal ligament.Membrane can be non-resorbable and resorbable. When non resorbable membranes are used, another surgical procedure for their extraction is needed. They are therefore used less frequently today. The majority of these membranes are made of polytetrafluoretylene, e.g. Gore-Tex membrane.Resorbable membranes shorten the treatment since there is no need for their removal. They can be made from natural materials like collagen, laminar bone, dura mater or connective tissue transplants and from synthetic resorbable materials, most frequently derivatives of organic aliphatic thermoplastic polymers. Polyglycolic and polylactic acids are mostly used. This group includes the Atrisorb membrane that has to be prepared intraoperatively. The use of polyurethane membranes is presently being tested.So far the perfect membrane has not been discovered. Collagen membranes are most popular due to their optimal biocompatibility, although their rate of resorption is difficult to predict. |

| Abstract: |
The results are shown of surgical-orthodontic treatment of developmental odontogenic cysts of the jaws with the purpose of demonstrating the importance of long-term teamwork of the orthodontist and oral surgeon in resolving each individual case, despite the fact that some cases may appear to be simple. Such cooperation enables the preservation of impacted teeth, even in cases when this appears almost impossible. The example presented of treatment of a large follicular cyst in an eight-year old girl is proof of the success of such cooperation. The authors also present contemporary knowledge on the treatment of odontogenic cysts, warn of the local invasive nature of odontogenic keratocysts and show the results of the application of resorptive granular tricalcium phosphate. |

| Abstract: |
She was born in Slavonski Brod on 30th November 1920. Matriculated in Slavonska Požega in 1939 and graduated at the School of Medicine in Zagreb in 1945. She later passed specialist examinations in Dental Medicine in 1951 and Maxillofacial Surgery in 1971. She defended her dissertation thesis entitled "Growth and Development of the Jaw in the Innate Cleft Lip and Palate", in 1967 and subsequent habilitation study entitled "Hemihypertrophic and Hemiatrophic Deformations of the Face and Facial Bones" in 1972. In 1944 she began work as a volunteer in the Clinic of Dental Medicine at Šalata, which later became the School of Dental Medicine in Zagreb and the Clinic of Maxillofacial and Oral Surgery. During the last few years of the Second World War she assisted in the treatment of numerous wounded people, who at that time occupied the whole of the Clinic at Šalata. She was engaged in maxillofacial surgery, dental medicine and anaesthesiology, and acted as an anaesthesiologist up until the arrival of Prof. Vera Arko to the Clinic in 1964. At the time when Ivo Čupar was preparing his life work "Surgery of the Head and Neck" in 3 volumes Prof. Pilar Svoboda helped with a large part of the work and publishing the books. After her retirement she was confronted with a disease which restricted her communication with people closest to her. As she was a dynamic person such a life was a particularly heavy burden, which at times was hard for her to bear. Fate prevented her enjoyment with Ivna and Mirna in the company of her grandchildren. |
